摘 要:目的:探讨莫匹罗星软膏联合紫外线对老年足部慢性溃疡患者胫骨横向骨搬移(tibial transverse transport,TTT)术后局部炎性因子和功能康复治疗效果的影响。方法:选取2019年8月—2021年8月本院就诊的老年足部慢性溃疡TTT术后患者68例作为研究对象,按照随机数字表法将其分为观察组和对照组,每组34例。对照组给予莫匹罗星软膏进行治疗,观察组在对照组治疗基础上联合紫外线治疗。比较两组创面愈合时间、住院时间、炎性因子变化[白介素-6(interleukin-6,IL-6)、C-反应蛋白(C-reactive protein,CRP)、肿瘤坏死因子-α(tumor necrosis factor,TNF-α)]、创面愈合面积及踝肱指数(ankle brachial index,ABI)。结果:观察组愈合时间、住院时间均低于对照组(P<0.05);治疗后,两组TNF-α、IL-6、CRP水平低于对照组,且观察组低于对照组(P<0.05);观察组患肢创面大小和ABI指标优于对照组(P<0.05)。结论:莫匹罗星软膏联合紫外线治疗老年足部慢性溃疡TTT术后患者的效果显著,可降低炎症反应,加快创面愈合,利于预后,值得推广应用。Objective: To investigate the effect of local inflammatory factors and functional rehabilitation in tibia(tibial transverse transport(TTT) in elderly patients with chronic foot ulcers. Methods: A total of 68 elderly patients with chronic foot ulcer after TTT in our hospital from August 2019 to August 2021 were selected as the research subjects. According to the random differentiation method, they were divided into observation group and control group, with 34 cases in each group. The control group was treated with mupirocin ointment, and the observation group was treated with ultraviolet radiation on the basis of the control group. The wound healing time, hospitalization time, inflammatory factors [ interleukin-6(IL-6), C-reactive protein(CRP), tumor necrosis factor-α(TNF-α) ], wound healing area and ankle brachial index(ABI) were compared between the two groups. Results: The healing time and hospitalization time of the observation group were lower than those of the control group, and the difference was significant(P<0.05). After treatment, the values of TNF-α, IL-6 and CRP in the two groups decreased compared with those before treatment, and the above hand indexes in the observation group were lower than those in the control group(P<0.05). The wound size and ABI index of the affected limb in the observation group were better than those in the control group, with significant difference(P<0.05). Conclusion: The effect of mupirocin ointment combined with ultraviolet ray in the treatment of elderly patients with chronic foot ulcer after TTT is remarkable, which can reduce the inflammatory response, accelerate wound healing, and is beneficial to prognosis. It is worthy of popularization and application.
