某院2019-2021年住院患者麻醉药品使用情况分析

摘　　要：目的 分析某院2019-2021年住院患者麻醉药品使用情况,加强使用麻醉药品的规范管理,保障麻醉药品在临床中合理使用。方法 采用厦门医学院附属第二医院的HIS系统对2019-2021年住院患者使用麻醉药品的使用情况进行抽调,统计、分析2019-2021年住院患者麻醉药品销售金额、全部药品销售金额及麻醉药品销售占比、麻醉药品使用数量与消耗量、销售金额与销售金额排序(B)、用药频度(DDDs)、限定日费用(DDC)以及药品B/DDDs排序(A)值。结果 目前该院有麻醉药品17种,其中注射剂有8种,口服片剂7种,贴剂2种。麻醉药品2019-2021年销售金额逐年略有增长,其在全部药品销售金额的占比也逐年略有上升。2020年、2021年住院患者使用麻醉药品DDDs前三位的均为舒芬太尼注射液、注射用瑞芬太尼(1 mg)、注射用瑞芬太尼(2 mg)。DDC排序前三位的是注射用瑞芬太尼(1 mg)、注射用瑞芬太尼(2 mg)、盐酸氢吗啡酮注射液。注射用瑞芬太尼(2 mg)、哌替啶注射液、芬太尼透皮贴(8.4 mg)、芬太尼透皮贴(4.2 mg)、吗啡片等药物的B/A值约等于1。结论 医院住院患者麻醉药品临床使用基本合理,临床能够严格执行三阶梯止痛原则,全面、认真地贯彻并落实麻醉药品相关法律法规,临床医师能够根据麻醉药品的适应证并结合患者具体病情个体化给药,规范使用麻醉药品,从而做到安全、合理用药。Objective To analyze the use of narcotic drugs by inpatients in a hospital from 2019 to 2021,in order to strengthen the standardized management of the use of narcotic drugs,and ensure the rational use of narcotic drugs in clinical practice. Methods Using the hospital’s HIS system,the use of narcotic drugs by inpatients in the Second Affiliated Hospital of Xiamen Medical College from 2019 to 2021 was selected,statistic and analyze the sales amount of narcotic drugs,total sales amount and proportion of narcotic drugs,the number and consumption of narcotic drugs,the order of sales amount and sales amount(B),DDDs,DDC results and the value of drug B/DDDs order(A) from 2019 to 2021. Results At present,the hospital had 17 kinds of narcotic drugs,included 8 kinds of injections,7 kinds of oral tablets and 2 kinds of patches. The sales amount of narcotic drugs increased slightly year by year from 2019 to 2021,and its proportion in the total sales amount of drugs also increased slightly year by year. Sufentanil injection,remifentanil for injection( 1 mg) and remifentanil for injection( 2 mg) were the top three drugs used in the inpatients. The top three DDC ranked remifentanil for injection( 1 mg),remifentanil for injection( 2 mg),and hydromorphone hydrochloride injection. The B/A value of remifentanil( 2 mg),pethidine injection,transdermal fentanyl( 8. 4 mg),transdermal fentanyl( 4. 2 mg) and morphine tablets for injection was about 1.Conclusion The clinical use of narcotic drugs for inpatients in a hospital is basically reasonable. Clinically,we should strictly implement the three-step analgesic principle,comprehensively and seriously implement and implement the laws and regulations related to narcotic drugs. Clinicians should individualize the administration of narcotic drugs according to the indications of narcotic drugs and the specific condition of patients,and standardize the use of narcotic drugs,so as to achieve safe and rational drug use.
