保妇康栓联合龙胆泻肝汤加减治疗宫颈炎并HPV感染的疗效及其对炎性反应的影响

摘　　要：目的 观察保妇康栓联合龙胆泻肝汤加减治疗宫颈炎并人乳头状瘤病毒(HPV)感染的临床疗效及其对炎性反应的影响。方法 选取2018年10月-2019年9月福建医科大学附属三明第一医院收治的宫颈炎并HPV感染患者86例,按照随机数字表法分为对照组与观察组,各43例。对照组予以保妇康栓治疗,观察组在对照组基础上予以龙胆泻肝汤加减治疗。2组均连续治疗3个月。比较2组临床疗效,症状消失时间、宫颈柱状上皮异位愈合时间与HPV-DNA转阴时间,治疗前及治疗3个月后血清炎性因子[白介素1β(IL-1β)、肿瘤坏死因子α(TNF-α)、C反应蛋白(CRP)]水平,不良反应发生率、随访3个月复发率。结果 观察组治疗总有效率为93.02%,高于对照组的74.42%(χ^(2)=5.460,P=0.019)。观察组症状消失时间、宫颈柱状上皮异位愈合时间、HPV-DNA转阴时间短于对照组(P <0.01)。治疗3个月后,2组血清IL-1β、TNF-α、CRP水平低于治疗前,且观察组低于对照组(P <0.01)。观察组不良反应总发生率为4.65%,与对照组的6.98%比较,差异无统计学意义(P=1.000)。观察组随访3个月复发率低于对照组(P <0.05)。结论 保妇康栓联合龙胆泻肝汤加减治疗宫颈炎并HPV感染的临床疗效确切,可有效缩短症状消退时间,降低血清炎性因子水平,从而减轻炎性反应,降低复发率,且安全性较高。Objective To analyze the clinical efficacy of Baofukang suppository combined with modified Longdanxiegan decoction on treating cervicitis with HPV infection and its influence on inflammatory reaction. Methods A total of86 cases of patients with cervicitis with HPV infection were selected from October 2018 to September 2019 in Sanming First Hospital Affiliated to Fujian Medical University,which were divided into control group and observation group according to the random number table method,with 43 cases in each group. The control group was treated with Baofukang suppository,while the observation group was treated with modified Longdanxiegan decoction on the basis of the control group. Both groups were treated continuously for 3 months. Clinical efficacy,symptom disappearance time,ectopic healing time of cervical columnar epithelium and HPV-DNA negative conversion time,serum inflammatory factors( IL-1β,TNF-α,CRP) before treatment and after 3 months of treatment,incidence of adverse reactions and recurrence rate after 3 months follow-up were compared between the two groups. Results The total effective rate of the observation group was 93. 02%,which was higher than74. 42% of the control group( χ^(2)= 5. 460,P = 0. 019). The symptom disappearance time,ectopic healing time of cervical columnar epithelium and HPV-DNA negative conversion time in the observation group were shorter than those in the control group( P < 0. 01). After 3 months of treatment,the levels of serum IL-1β,TNF-α and CRP in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group( P < 0. 01). The incidence of adverse reactions in the observation group was 4. 65%,compared with 6. 98% in the control group,the difference was not statistically significant( P > 0. 05). The recurrence rate after 3 months follow-up of observation group was lower than that of control group( P < 0. 05).
