摘  要:目的 观察肺表明活性物质(PS)联合枸橼酸咖啡因注射液及无创机械通气治疗新生儿呼吸窘迫综合征(NRDS)的临床疗效。方法 选取2019年1月-2021年1月信宜市人民医院收治的NRDS患儿60例,采用随机数字表法分为对照组和试验组,每组30例。患儿均接受无创机械通气治疗,在此基础上,对照组患儿予以PS猪肺磷脂注射液治疗,试验组患儿在对照组基础上予以枸橼酸咖啡因注射液治疗,2组均持续治疗7 d。比较2组临床疗效,吸氧时间、机械通气时间、住院时间、呼吸暂停次数,治疗前及治疗7 d后呼吸功能指标[呼吸频率(RR)、潮气量(VT)、吸气时间(Ti)、吸气时间与呼气时间比值(Ti/Te)],并发症发生情况。结果 试验组总有效率为96.67%,高于对照组的73.33%(χ^(2)=4.706,P=0.030)。试验组吸氧时间、机械通气时间、住院时间短于对照组,呼吸暂停次数少于对照组(P <0.01)。治疗7 d后,2组RR、Ti、Ti/Te低于治疗前,VT高于治疗前,且试验组RR、Ti、Ti/Te低于对照组,VT高于对照组(P <0.01)。试验组并发症总发生率为6.67%,与对照组的10.00%比较,差异无统计学意义(P=1.000)。结论 PS联合枸橼酸咖啡因注射液及无创机械通气治疗NRDS的临床疗效确切,可缩短患儿治疗时间,改善呼吸功能,进而缓解症状,且安全性较高。Objective To explore the clinical effect of pulmonary surfactant( PS) combined with caffeine citrate and noninvasive mechanical ventilation in the treatment of neonatal respiratory distress syndrome( NRDS). Methods Sixty children with NRDS treated in Xinyi City People’s Hospital from January 2019 and January 2021 were selected as the research objects,and they were divided into a control group and experimental group,with 30 cases in each group. All children were treated with noninvasive mechanical ventilation,on the basis of this,control group was treated with PS of pig pulmonary phospholipid injection,and the experimental group was treated with caffeine citrate injection. Both groups were treated with7 days. Clinical effect,oxygen inhalation time,mechanical ventilation time,hospitalization time,number of apnea,respiratory function indexes( RR,VT,Ti,Ti/Te) before treatment and after 7 days of treatment,incidence of complications were compared between the two groups. Results Total effective rate of experimental group was 96. 67%,which was higher than73. 33% of control group( χ^(2)= 4. 706,P = 0. 030). Oxygen Inhalation time,mechanical ventilation time,hospitalization time of experimental group were shorter than those of control group,number of apnea was less than that of control group( P <0. 01). After 7 days of treatment,RR,Ti,Ti/Te in the two groups were lower than those before treatment,VT was higher than that before treatment,and RR,Ti,Ti/Te of experimental group were lower than those of control group,VT was higher than that of control group( P < 0. 01). There was no significant difference of incidence of complications between experimental group( 6. 67%) and control group( 10. 00%)( P = 1. 000). Conclusion PS combined with caffeine citrate and noninvasive mechanical ventilation have an exact clinical effect in the treatment of NRDS,can shorten treatment time,improve respiratory function,and then relieve symptoms of children,and with higher safety.
