基于“动静平衡”原则探析脊髓型颈椎病诊断及正脊治疗

摘　　要：脊髓型颈椎病是颈椎病最严重的亚型,能否进行正脊手法治疗、如何选择恰当的正脊手法一直是脊髓型颈椎病治疗的争论焦点。张俊忠教授提出的“动静平衡”原则强调骨折术后固定与功能锻炼要保持相对平衡的关系。在脊髓型颈椎病的诊治中,以该原则为基础,重视颈椎解剖结构和生物力学方面的“动静平衡”,准确评估疾病是处于急性进展期还是慢性平稳期,根据不同分期的病情特点确定理筋类手法和矫正类手法的应用方案,可避免医源性损伤,提高治疗的安全性和有效性。Cervical spondylotic myelopathy is the most severe subtype of cervical spondylosis.Whether chiropractic treatment could be applied and how to choose appropriate chiropractic manipulation have always been the controversy points of treatments of cervical spondylotic myelopathy.The“dynamic and static balance”principle proposed by Professor ZHANG Junzhong emphasizes the balance between fracture fixation and functional exercise after fracture surgery.In the diagnosis and treatment of cervical spondylotic myelopathy,based on the“dynamic and static balance”principle proposed by Professor ZHANG Junzhong,the“dynamic and static balance”in anatomical structure and biomechanics of cervical vertebra also should be placed a high value and thus whether the disease is in an acute progression stage or a chronic plateau stage can be accurately assessed.Making application scheme of tendon-regulation manipulation and correction manipulation according to the disease characteristics of different stage can avoid iatrogenic injuries and improve the efficacy and safety of the treatment.
