剖宫产瘢痕妊娠术前高强度超声聚焦治疗的临床研究

摘　　要：目的探究剖宫产瘢痕妊娠(CSP)术前采用高强度超声聚焦治疗的临床效果。方法回顾性分析于本院明确诊断为CSP的30例患者的临床资料,将15例采用常规药物联合宫腔镜下子宫瘢痕处妊娠物清除术治疗者纳入对照组,将15例采用宫腔镜下子宫瘢痕处妊娠物清除术联合术前高强度超声聚焦治疗者纳入研究组。对比两组疗效、术中出血量、血清β-人绒毛促性腺激素(β-HCG)水平复常时间及月经复常时间。结果研究组治疗总有效率为93.33%,高于对照组的66.67%,但两组间比较,差异无统计学意义(P>0.05)。与对照组相比,研究组术中出血量较少,血清β-HCG水平复常时间、月经复常时间均较短,差异有统计学意义(P<0.05)。结论CSP患者术前采用高强度超声聚焦治疗的效果显著,此预处理方式可降低术后出血风险,促进子宫生理功能尽快复常。Objective To investigate the clinical effect of high intensity ultrasound focused therapy before cesarean scar pregnancy(CSP).Methods The clinical data of 30 patients diagnosed as CSP in our hospital from August 2021 to February 2022 were retrospectively analyzed.15 patients treated with conventional drugs combined with hysteroscopic pregnancy removal from uterine scar were included in the control group,and 15 patients treated with hysteroscopic pregnancy removal from uterine scar combined with preoperative high intensity ultrasound focused therapy were included in the study group.The efficacy,intraoperative blood loss,serum β-human chorionic gonadotropin(β-HCG)level normalization time and menstrual normalization time were compared between the two groups.Results The total effective rate of the study group was 93.33%,which was higher than that of the control group 66.67%,but there was no significant difference between the two groups(P>0.05).Compared with the control group,the intraoperative blood loss,serum β-HCG level normalization time and menstrual normalization time in the study group were less,and the differences were statistically significant(P<0.05).Conclusion The effect of high-intensity ultrasound focused therapy is significant in patients with CSP before operation,which can reduce the risk of postoperative bleeding and promote the recovery of uterine physiological function as soon as possible.
