经阴道彩超对未破裂型宫外孕超声诊断的价值分析

摘　　要：目的探究经阴道彩超对未破裂型宫外孕超声诊断的临床价值。方法选取吉林普济医院收治的96例疑似未破裂型宫外孕患者为研究对象,均接受经腹部彩超、经阴道彩超检查,分析经阴道彩超对未破裂型宫外孕患者的临床诊断价值。结果96例患者经手术病理诊断确诊阳性54例、阴性42例。经阴道彩超诊断准确度、敏感度、特异度分别为89.58%、88.89%、90.48%,均高于经腹部彩超的68.75%、68.52%、69.05%,差异有统计学意义(P<0.05);与经阴道彩超联合经腹部彩超的95.83%、94.44%、97.62%比较,差异无统计学意义(P>0.05)。经阴道彩超联合经腹部彩超对胚芽、假孕囊、盆腔积液、附件包块诊断检出率分别为46.88%、32.29%、43.75%、34.38%,与经阴道彩超的38.54%、30.21%、40.63%、31.25%比较,差异无统计学意义(P>0.05);二者均高于经腹部彩超的9.38%、19.79%、21.88%、14.58%,差异有统计学意义(P<0.05)。结论在未破裂型宫外孕的超声诊断中,与经腹部彩超相比,应用经阴道彩超可提升诊断效能,且两种超声联合诊断可减少漏诊、误诊患者数量。Objective To explore the clinical value of transvaginal color Doppler ultrasound in the diagnosis of unruptured ectopic pregnancy.Methods A total of 96 patients with suspected unruptured ectopic pregnancy admitted to Jilin Puji Hospital were selected as the research objects.All of them were examined by transabdominal color Doppler ultrasound and transvaginal color Doppler ultrasound.The clinical diagnostic value of transvaginal color Doppler ultrasound in unruptured ectopic pregnancy was analyzed.Results Among 96 patients,54 cases were positive and 42 cases were negative.The diagnostic accuracy,sensitivity and specificity of transvaginal ultrasound were 89.58%,88.89% and 90.48% respectively,which were higher than 68.75%,68.52% and 69.05% of transabdominal ultrasound,the differences were statistically significant(P<0.05).The detection rates of coleophenia,pseudogestational sac,pelvic effusion and adjunctive mass of transvaginal ultrasound combined with transabdominal ultrasound were 46.88%,32.29%,43.75% and 34.38%,respectively,compared with 38.54%,30.21%,40.63% and 31.25% of transvaginal ultrasound,there were no significant differences(P>0.05);but both of them were higher than 9.38%,19.79%,21.88%,14.58% of transabdominal ultrasound,and the differences were statistically significant(P<0.05).Conclusion Compared with transabdominal color Doppler ultrasound,transvaginal color Doppler ultrasound can improve the efficiency in the diagnosis of unruptured ectopic pregnancy,and the combined diagnosis of two kinds of Doppler ultrasound can reduce the number of missed diagnosis and misdiagnosis.
