围生期综合护理对初产妇母乳分泌及喂养情况的影响

摘　　要：目的探讨围生期综合护理对初产妇母乳分泌及喂养的影响。方法选择初产妇80例,按照随机数字表法分为对照组和研究组,每组40例。对照组实施常规护理,研究组在常规护理基础上实施围生期综合护理。统计初产妇产后泌乳时间,产后24、48、72 h的泌乳量评分以及母乳喂养情况。结果研究组初产妇在产后<24、24~48 h泌乳例数均多于对照组,产后48-72,>72 h泌乳例数均少于对照组,差异均有统计学意义(P<0.05)。研究组初产妇产后24、48、72 h泌乳量评分均高于对照组,差异均有统计学意义(P<0.05)。研究组初产妇纯母乳喂养率高于对照组,人工喂养率低于对照组,差异均有统计学意义(P<0.05);两组混合喂养率比较差异无统计学意义(P>0.05)。结论在初产妇分娩期间实施围生期综合护理,可缩短初产妇产后泌乳时间,增加泌乳量,提高纯母乳喂养率,值得临床推广与应用。Objective To explore the effect of perinatal comprehensive nursing on breast milk secretion and feeding of primipara.Methods Eighty primiparas were selected as the research objects.According to the random number table method,primiparas were divided into control group and study group,40 cases in each group.The control group received routine nursing,while the study group received perinatal comprehensive nursing on the basis of routine nursing.The lactation time,lactation score at 24,48 and 72 h after delivery and breastfeeding status of primiparas were recorded.Results The number of lactating cases in the study group at postpartum<24 h and 24-48 h was higher than that in the control group,and the number of lactating cases at postpartum 48-72 and>72 hours was lower than that in the control group,and the differences were statistically significant(P<0.05).The lactation scores of primiparas in the study group at 24,48 and 72 hours postpartum were higher than those in the control group,and the differences were statistically significant(P<0.05).The exclusive breastfeeding rate of primiparas in the study group was higher than that in the control group,and the artificial feeding rate was lower than that in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the mixed feeding rate between the two groups(P>0.05).Conclusion Perinatal comprehensive nursing during clinical delivery of primiparas can shorten the postpartum lactation time of primiparas,increase lactation volume and improve the rate of exclusive breastfeeding,which is worthy of clinical promotion and application.
