清热活血方对不稳定型心绞痛患者MMP-9及hs-CRP的影响

摘　　要：目的:临床观察清热活血方对热毒血瘀证不稳定型心绞痛(UA)患者的疗效及对基质金属蛋白酶-9(MMP-9)、高敏C反应蛋白(hs-CRP)、TIMI风险评分的影响。方法:将60例UA患者随机分成对照组和治疗组,每组各30例,2组均予西医标准治疗,治疗组加用清热活血方(由连翘、黄连、生地、丹参、赤芍、红花、川芎等组成),疗程均为2周。评价两组UA患者治疗后的临床疗效,比较两组患者治疗前后MMP-9、hs-CRP水平及TIMI评分的变化。结果:治疗组中医证候疗效总有效率86.7%高于对照组的63.3%(P<0.05)。治疗后MMP-9水平,2组内及组间均有统计学差异(P<0.01,P<0.05);治疗后hs-CRP水平,2组内比较有统计差异(P<0.05)。治疗后TIMI评分,2组内比较有统计学意义(P<0.05)。结论:清热活血方可以改善UA患者的临床症状,降低MMP-9、hs-CRP的水平及TIMI评分。Objective:to clinically observe the efficacy of heat clearing and invigorating blood formula on patients with unstable angina pectoris(UA)with heat,toxin and blood stasis syndrome and its effect on matrix metalloproteinase-9(MMP-9),high-sensitivity C-reactive protein(hs-CRP),TIMI risk score.Methods:a total of 60 patients with UA were randomly divided into a control group and a treatment group of 30 patients each,and two groups were given the standard treatment of Western medicine,with the addition of a heat clearing and invigorating formula(composed of forsythia,Coptis chinensis,radix rehmanniae,Salvia miltiorrhiza,Radix rubra,safflower,and Chuanxiong)to the treatment group,each for 2 weeks.To evaluate the clinical efficacy of two groups of UA patients after treatment,the levels of MMP-9,hs-CRP and TIMI scores before and after treatment were compared between the two groups.Results:the total effective rate of 86.7% of TCM syndrome cases in the treatment group was higher than the 63.3% in the control group(P<0.05).MMP-9 levels after treatment,both within and between the 2 groups(P<0.01,P<0.05);Levels of hs-CRP after treatment,and there were statistical differences between the 2 groups(P<0.05).TIMI scores after treatment,the comparison within the 2 groups was statistically significant(P<0.05).Conclusions:the heat clearing and activating blood formula can improve the clinical symptoms of UA patients and reduce the levels of MMP-9,hs-CRP and TIMI scores.
