杨勤从肝脾论治肠易激综合征的临床经验

摘　　要：肠易激综合征是临床上常见的功能性胃肠疾病,归属于中医的“泄泻”“腹痛”“便秘”等病症范畴。其病位在肠,与肝脾相关密切。肠易激综合征虽不影响患者的长期生存,但却对患者的生活和工作造成了极大的影响。目前西医对IBS的治疗效果难以让患者满意。肝脾二脏作为人体的两大消化器官,在消化系统疾病的发生和治愈过程中发挥着巨大的作用。杨勤教授在传承金陵医派重视顾护脾胃的学术思想基础上,根据肝脾的相互联系以及肝脾二脏在IBS发病的密切相关性的认识上,在临床上从肝脾辨证施治,采用疏肝健脾、抑肝扶脾、养肝补脾之法来治疗肠易激综合征,同时依据“知犯何逆,随证治之”的原则,灵活使用中药进行加减,以对症治疗。该文通过总结杨勤教授从肝脾辨证论治肠易激综合征的临床经验,以期为临床治疗肠易激综合征提供参考。Irritable bowel syndrome (IBS)is a common functional gastrointestinal disease in clinic,which belongs to “diarrhea”,“abdominal pain”,“constipation” and other diseases in Chinese medicine.It is located in the intestine and closely related to the liver and spleen.Although irritable bowel syndrome does not affect the long-term survival of patients,it has a great impact on the life and work of patients.At present,the treatment effect of western medicine on IBS is difficult to satisfy patients.As the two digestive organs of the human body,the liver and spleen play a huge role in the occurrence and cure of digestive diseases.Professor YANG Qin,on the basis of inheriting the academic thought of Jinling Medical School that pays attention to the care of the spleen and stomach,and on the basis of the understanding of the interrelationship between the liver and spleen and the close correlation between the liver and the spleen in the pathogenesis of IBS,has clinically treated IBS from the perspective of liver and spleen syndrome differentiation,using the methods of soothing the liver and strengthening the spleen,inhibiting the liver and supporting the spleen,and nourishing the liver and spleen.At the same time,according to the principle of “knowing what is wrong,and treating it according to the syndrome”,he has flexibly used traditional Chinese medicine to add and subtract,Treat symptomatically.This article summarized Professor YANG Qin’s clinical experience in treating irritable bowel syndrome based on liver and spleen syndrome differentiation,hoping to provide reference for clinical treatment of irritable bowel syndrome.
