摘  要:目的探讨T2DM患者睡眠时长、睡眠规律性与HbA_(1)c的相关性。方法选取2018年7~11月于北京市通州区、顺义区4个社区卫生服务中心及下属22个社区卫生服务站招募符合条件的T2DM患者388例。采用智能手环测量睡眠时长和睡眠规律性,按照睡眠时长(<6、6~、7~、8~、≥9 h/d)和睡眠规律性(<2、2~、3~、≥4 h/d)分组,采用HbA_(1)c评估血糖控制状况。多因素线性回归分析HbA_(1)c的影响因素。结果睡眠时长为(7.6±1.5)h/d,睡眠规律性为(3.5±1.8)h/d。调整混杂因素后睡眠时长过短(<6 h/d)者HbA1c是睡眠时长正常(7~h/d)者的1.09倍(P<0.01);睡眠不规律(3~、≥4 h/d)者HbA_(1)c是睡眠较规律(<2 h/d)者的1.06倍(P=0.05)和1.08倍(P<0.01)。按照性别和年龄分层后上述关联仅在女性和<60岁患者中存在。结论T2DM患者睡眠时长过短、睡眠不规律是HbA1c升高的影响因素,尤其是女性和<60岁患者。Objective To investigate the association of sleep duration and sleep regularity with HbA_(1)c level in patients with type 2 diabetes mellitus(T2DM).Methods A cross-sectional study was conducted among 388 T2DM patients,from 22 community health service stations of 4 community health service centers in Tongzhou and Shunyi district in Beijing between July and November 2018.Sleep duration and Sleep regularity were measured by wearable activity trackers.All the participants were divided into groups by sleep duration(<6,6~,7~,8~and≥9 h/d)and sleep regularity(<2,2~,3~and≥4 h/d).The glycemic control status was assessed by HbA_(1)c.Influencing factors for HbA_(1)c were accessed by multivariate linear regression analysis.Results The sleep duration of the participants was(7.6±1.5)h/d,and the sleep regularity was(3.5±1.8)h/d.After multivariable adjustment,the HbA_(1)c levels were 1.09 times in patients with shortest(<6 h/d sleep durations,P<0.01)than in patients with normal sleep durations(7~h/d).The HbA_(1)c levels were 1.06 times(P=0.05)and 1.08 times(P<0.01)in patients with irregular sleep patterns(3~,≥4/d)that in patients with regular sleep patterns(<2 h/d).Stratified by sex and age,the above associations persisted only in women and in people<60 years old.Conclusion Short sleep duration and sleep irregularity were associated with high HbA_(1)c levels,especially in women and in patients<60 years old.
