摘 要:目的 探讨新生儿支气管肺发育不良(BPD)患儿Toll样受体10(TLR-10)、肺泡表面活性物质蛋白A1(SP-A1)基因多态性及其与病情程度的关系。方法 选取承德市中心医院83例BPD患儿作为观察组,另选取83例同期健康新生儿作为对照组。比较两组血清TLR-10、SP-A1水平、TLR-10rs11096955位点、SP-A1AA50位点基因多态性,分析两者基因多态性对BPD易感性的影响及与病情程度的关系。结果 观察组血清TLR-10、SP-A1水平高于对照组(P<0.05);TLR-10 rs11096955位点、SP-A1 AA50位点基因型分布具有人群代表性。观察组与对照组TLR-10 rs11096955位点、SP-A1 AA50位点基因型、等位基因频率差异有统计学意义(P<0.05);TLR-10 rs11096955位点AA基因型BPD易感性是AC/CC的1.401倍;SP-A1 AA50位点GG基因型BPD易感性是GC/CC的1.692倍;TLR-10 rs11096955位点AA基因型、等位基因A频率、SP-A1 AA50位点GG基因型、等位基因G频率与BPD病情程度呈正相关(P<0.05)。结论 TLR-10 rs11096955位点AA基因型、SP-A1 AA50位点GG基因型会显著增加新生儿BPD易感性,且与病情程度密切相关。Objective To explore the relationship between Toll-like receptor 10(TLR-10) and alveolar surfactant protein A1(SP-A1) in the cases of children with neonatal bronchopulmonary dysplasia(BPD). Methods A total of 83 children with BPD in Chengde Central Hospital were selected as the observation group, and 83 healthy newborns during the same period were selected as the control group. The serum levels of TLR-10, SP-A1, TLR-10 rs11096955 locus and SP-A1 AA50 locus gene polymorphism were compared between the two groups, and the effect of the two gene polymorphisms on the susceptibility of BPD and the relationship with the disease severity were analyzed. Results The serum levels of TLR-10 and SP-A1 in the observation group were higher than those in the control group(P<0.05). The genotype distribution of TLR-10 rs11096955locus and SP-A1 AA50 locus was representative of the population. There were significant differences in the genotype and allele frequency of TLR-10 rs11096955 locus and SP-A1 AA50 locus between the observation group and the control group(P<0.05). TLR-10 rs11096955 locus AA genotype BPD susceptibility was AC/CC 1.401 times;SP-A1 AA50 locus GG genotype BPD susceptibility is 1.692 times higher than GC/CC. TLR-10 rs11096955 locus AA genotype, allele A frequency, SP-A1AA50 locus GG gene type and allele G frequency were positively correlated with the severity of BPD(P<0.05). Conclusion TLR-10 rs11096955 locus AA genotype, SP-A1 AA50 locus GG genotype can significantly increase the susceptibility of neonatal BPD, and is closely related to the degree of disease.
