养阴清热活血法治疗干燥综合征的临床舌象研究

摘　　要：目的:观察养阴清热活血法对阴虚内热血瘀型原发性干燥综合征患者舌象的影响。方法:临床采用非盲法随机对照设计,将91例阴虚内热血瘀型原发性干燥综合征患者随机分为对照组45例和观察组46例。观察组口服养阴清热活血法基本方的中药汤剂,对照组口服硫酸羟氯喹片和白芍总苷胶囊。治疗12、24周时,观察比较治疗前后两组数字化舌象特征值的变化。结果:两组患者性别、年龄、病程等基线指标比较差异无统计学意义(P>0.05)。在数字化舌象特征值的改善方面,治疗后观察组舌色和苔色的R(红色)、V(明度)、胖瘦特征值水平均较对照组有显著改善(P<0.05),且舌色和苔色的R(红色)、G(绿色)、B(蓝色)、V(明度)、舌苔厚薄、裂纹、舌体胖瘦特征值均较治疗前有显著改善(P<0.05)。治疗时间的延长对观察组在以上各观察指标的改善上均有积极意义。结论:养阴清热活血法能明显改善阴虚内热血瘀型原发性干燥综合征患者的数字化舌象特征值,临床疗效良好。Objective:To observe the influence of Yin-nourishing heat-clearing blood-activating method on tongue manifestations in patients with primary Sjogren's syndrome of Yin deficiency,internal heat and blood stasis pattern.Methods:By adopting unblinded randomized controlled design,91 patients were randomized into 45 cases of the control group and 46 cases of the observation group.The observation group took the basic formulation of Yin-nourishing heat-clearing blood-activating method,and the control group orally hydroxy-chloroquine sulfate tablets and total glucosides of paeony capsules.To observe and compare the changes of digital tongue image features before and after treatment when both groups were treated for 12 and 24 weeks.Results:The results of clinical observation showed that the difference had no statistical meaning in the baseline indicators including gender,age and the course of the disease between the observation group and the control group(P>0.05).In terms of the improvement of digital tongue image features,the tongue color and the colors of tongue coating such as redness(R),value(V),plumpness and slenderness of tongue of the observation group were improved remarkably compared with these of the control group(P<0.05),meanwhile the tongue color and the colors of tongue coating including R,G,B and V,the thickness of tongue coating,cracked tongue,plumpness and slenderness of tongue were improved than these before treatment(P<0.05).More therapeutic time was positive for the improvements of the above indexes in the observation group.Conclusion:Yin-nourishing heat-clearing blood-activating method could obviously improve digital tongue image features in patients with primary Sjogren's syndrome of Yin deficiency,internal heat and blood stasis pattern,and it could obtain good clinical effects.
