基于IKAP理论穴位敷贴辅助在功能性消化不良患者中的护理应用

摘　　要：目的:探讨穴位基于信息-知识-信念-行为(information-knowledge-attitude-practice,IKAP)理论敷贴辅助在功能性消化不良患者中的护理应用效果。方法:选取功能性消化不良患者360例,采用数字奇偶法分为观察组与对照组,各180例。对照组采用常规护理干预方法,观察组采用基于IKAP理论穴位敷贴辅助的护理模式进行干预。两组共干预4周,观察两组患者临床疗效,干预前后临床症状及胃泌素、免疫球蛋白A(immunoglobulin A,IgA)、免疫球蛋白M(immunoglobulin M,IgM)水平和生活质量指数问卷(quality of life index,Ql-INDEX)评分。结果:总有效率观察组[94.44%(170/180)]明显高于对照组[83.33%(150/180)](P<0.05);干预后观察组呕吐、腹痛、嗳气、腹胀症状积分明显低于对照组(P<0.05);干预后观察组胃泌素(gastrin,GAS)及IgA水平明显高于对照组(P<0.05),IgM低于对照组(P<0.05);干预后观察组生活质量评分明显高于对照组(P<0.05)。结论:穴位敷贴辅助基于IKAP理论的护理模式可减轻功能性消化不良患者的临床症状,提高胃泌素水平,同时还可改善免疫学指标,提高患者生活质量。Objective:To explore the effects of acupoint application supplemented with nursing model on patients with functional dyspepsia based on IKAP theory.Methods:Altogether 360 FD patients were chosen and allocated to the observation group and the control group by using number parity method,180 cases in each group.The control group adopted conventional nursing intervention method,and the observation group nursing model assisted by acupoint application based on IKAP theory.Both groups were intervened for four weeks,to observe clinical effects between both group,clinical symptoms before and after the intervention,the levels of gastrin(GAS),IgA,IgM and Q1-INDEX scores.Results:Total effective rate of the observation group was[94.44%(170/180)],clearly higher than[83.33%(150/180)]of the control group(P<0.05);after the intervention,the observation group was lower than the control group in the scores of vomiting,abdominal pain,belching and abdominal distension(P<0.05);after the intervention,the levels of GAS and IgA of the observation group were higher than these of the control group distinctly(P<0.05);the level of IgM lower than that of the control group(P<0.05);after the intervention,the observation group was higher than the control group in Q1-INDEX apparently(P<0.05).Conclusion:Acupoint application supplemented with nursing model based on IKAP could relieve clinical symptoms in FD patients,raise the levels of GAS,meanwhile it could improve immunological indicators and raise quality of life.
