发散式冲击波联合揿针埋针治疗钙化性冈上肌腱炎的疗效

摘　　要：目的探讨发散式冲击波联合揿针埋针治疗钙化性冈上肌腱炎的疗效。方法将杭州市萧山中医院2015年1月至2020年1月收治的钙化性冈上肌腱炎80例患者采用随机数字表法分为对照组(n=38)与观察组(n=42),两组均予发散式冲击波治疗,观察组在此基础上联合揿针埋针,记录并比较两组治疗前后疼痛视觉模拟评分(visual analog score,VAS)、Constant肩关节评分、加州大学肩关节评分量表(the university of California at Los Angeles shoulder rating scale,UCLA)变化,并比较两组钙化灶变化情况及两组患者治疗总有效率。结果治疗后两组患者的VAS评分较治疗前明显降低,差异有统计学意义(P<0.05);观察组患者的治疗后VAS评分较对照组改善更明显,差异有统计学意义(P<0.05);两组患者的治疗后Constant肩关节评分及UCLA肩关节评分均较治疗前明显提高,差异有统计学意义(P<0.05);且观察组患者的Constant肩关节评分及UCLA肩关节评分较对照组改善更明显,差异有统计学意义(P<0.05);观察组患者治疗后的有效率为95.2%,显著高于对照组的81.6%,差异有统计学意义(P<0.05)。结论发散式冲击波联合揿针埋针治疗钙化性冈上肌腱炎,可有效减轻患者的疼痛,改善肩关节功能。Objective To evaluate the efficacy of divergent shock wave combined with thumbtack needle in the treatment of calcified tendinitis of the supraspinatus.Methods A total of 80 patients with calcified supraspinatus tendinitis were included from January 2015 to January 2020 in Traditional Chinese Medical Hospital of Xiaoshan.According to random number table,the patients were divided into control group(n=38)and observation group(n=42).Both groups were treated with divergent shock wave,and the observation group was combined with thumbtack needle and embedding on base of the control group.The changes of visual analogue score(VAS),Constant shoulder joint score and the University of California at Los Angeles(UCLA)shoulder rating scale before and after treatment were recorded and compared between the two groups.The changes of calcified lesions and the total effective rate in the two groups were compared.Results After treatment,the VAS scores of the patients in the control group and the observation group decreased compared with those before treatment,with statistically significant difference(P<0.05).The VAS scores of the observation group improved more significantly after treatment than those of the control group,with statistically significant difference(P<0.05);The Constant score and UCLA score were significantly higher than those before treatment,with statistically significant difference(P<0.05);The Constant score and UCLA score of the shoulder joint in the observation group were significantly improved compared with those in the control group,with statistically significant difference(P<0.05);The total effective rate was 95.2%in the observation group,which was significantly higher than 81.6%in the control group,with statistically significant difference(P<0.05).Conclusion The treatment of calcified tendinitis of the supraspinatus by divergent shock wave combined with thumbtack needle can effectively relieve pain and improve the function of shoulder joint.
