摘  要:6%羟乙基淀粉130/0.4(6%hydroxyethyl starch 130/0.4,HES)是第三代羟乙基淀粉,中等分子量,输入体内后能够增加血管内的胶体渗透压,起到扩容作用。患者在严重创伤、感染应激和手术治疗后常出现低蛋白血症,严重影响机体功能,严重时可危及患者生命。传统观点认为以上低蛋白血症是肝脏蛋白合成不足和机体蛋白分解代谢加快导致的,但目前研究认为与微血管蛋白渗漏密切相关。最新研究发现,HES可以减少蛋白的微血管渗漏,其机制与抗炎作用有关。本文从HES在炎症反应、炎性通路的作用,对创伤性蛋白微血管渗漏的影响方面作一综述。6%hydroxyethyl starch 130/0.4(HES),the third generation of hydroxyethyl starch,is a medium molecular weight hydroxyethyl starch,which can increase the colloidal osmotic pressure in the blood vessels after infusion into the body and play a role in capacity expansion.Patients often develop hypoproteinemia after severe trauma,infectious stress,and surgery,which can seriously affect the function of the body,and it can endanger the patient's life in severe cases.The traditional view is that hypoproteinemia is caused by insufficient liver protein synthesis and accelerated protein catabolism in the body,but this is believed in current research to be closely related to microvessel protein leakage.The latest research has found that 6%HES can reduce the microvessel leakage of proteins,and its mechanism is related to anti-inflammatory effects.We review the effects of HES on the leakage of traumatic protein microvessels through inflammatory responses and inflammatory pathways.
