高电压脉冲射频治疗合并糖尿病的胸带状疱疹后神经痛的临床疗效

摘　　要：目的:观察胸背根神经节高电压脉冲射频治疗合并糖尿病的胸带状疱疹后神经痛的临床疗效。方法:选取60例经高电压脉冲射频治疗PHN患者,按照糖尿病控制情况进行分组,每组各20例:A组:无糖尿病,B组:血糖良好控制组,糖化血红蛋白控制在7%以内,C组:血糖控制不佳组,糖化血红蛋白控制在7%~10%之间。记录治疗前、治疗后的24h、1周、1个月、3个月、6个月各组患者的疼痛评分(VAS);分别对各组治疗前,治疗后1个月、3个月、6个月的PSQI、SF-36、焦虑抑郁评分、治疗后不良反应发生情况予以记录。结果:术后各组各时间点VAS明显降低(P<0.05),其睡眠质量、生活质量、情绪均明显改善(P<0.05);A组与B组在治疗后3个月、治疗后6个月VAS评分比较差异有统计学意义(P<0.05),C组与A组、B组在治疗后1周、治疗后1个月、治疗后3个月、治疗后6个月VAS评分比较差异有统计学意义(P<0.05);A组与B组在治疗后3个月、治疗后6个月PSQI、SF-36、SAS/SDS评分比较差异有统计学意义(P<0.05),C组与A组、B组在治疗后1个月、治疗后3个月、治疗后6个月PSQI、SF-36、SAS/SDS评分比较差异有统计学意义(P<0.05),整个治疗过程无严重并发症发生。结论:高电压脉冲射频对无糖尿病的PHN患者临床疗效确切、安全。Objective:To observe the clinical effect of high voltage pulse radiofrequency neuralgia combined with diabetes.Methods:60 patients with PHN by high voltage pulse radiofrequency were selected and grouped according to diabetes control.20 patients in each group:group A:no diabetes,group B:control group with good glucose control,HbA 1 c controlled within 7%,group C:poor glucose control,HbA 1 c controlled between 7-10%.The pain scores(VAS)of patients in each group were recorded before and 24 hours,1 week,1 month,3 months and 6 months after treatment;PSQI,SF-36,anxiety and depression scores,and adverse reactions after treatment in each group were recorded before treatment,1 month,3 months,and 6 months after treatment.Results:After operation,VAS decreased significantly at each time point in each group(P<0.05),and their sleep quality,life quality and mood improved significantly(P<0.05);There was a statistically significant difference in VAS scores between group A and group B at 3 months and 6 months after treatment(P<0.05),and there was a statistically significant difference in VAS scores between group C and group A and group B at 1 week,1 month,3 months and 6 months after treatment(P<0.05);There were significant differences in PSQI,SF-36,SAS/SDS scores between group A and group B at 3 months and 6 months after treatment(P<0.05).There were significant differences in PSQI,SF-36,SAS/SDS scores between group C and group A and group B at 1 month,3 months and 6 months after treatment(P<0.05).There were no serious complications during the whole treatment process.Conclusion:High field strength RF is effective and safe in PHN and patients with good blood glucose control.
