数字减影血管造影机造影诊断与CT诊断在主动脉夹层患者中的运用对比

摘　　要：目的:分析数字减影血管造影机造影诊断与CT诊断在主动脉夹层患者中的诊断价值。方法:选取2019年6月~2022年6月本院治疗的60例疑似主动脉夹层患者为研究对象,所有患者均接受数字减影血管造影机造影诊断与CT诊断,对比两种方式真假腔数量和累及髂动脉、肠系膜动脉、肾动脉情况以及主动脉第一破口位置、夹层血栓显示情况。结果:诊断试验分析数字减影血管造影机造影结果作为金标准,CT诊断的灵敏度和特异度为100.0%(57/57)、100%(3/3);两种检查方式在主动脉破裂数量、主动脉第一破口位置、真假腔数量相比差异无统计学意义(P>0.05);同时,两种诊断方式累及髂动脉、肠系膜动脉、肾动脉情况相比无统计学差异(P>0.05);数字减影血管造影机造影无法显示夹层血栓,CT诊断有14例夹层血栓,夹层血栓显示率为23.33%,CT对夹层血栓的显示率显著高于数字减影血管造影机造影诊断(P<0.05)。CT检查图像可见胸主动脉双腔主动脉、夹层累及左侧髂总动脉、夹层真假腔,数字减影血管造影机造影图像在两腔之间见线状分隔透亮影。结论:在主动脉夹层诊断中采用CT诊断方式可获得比数字减影血管造影机造影更高的诊断准确率,尤其夹层血栓显示方面具有更大的优势。Objective:To analyze the diagnostic value of digital subtraction angiography(DSA)and CT in patients with aortic dissection.Methods:60 patients suspected of aortic dissection treated in our hospital from June 2019 to June 2022 were selected as the study objects.All patients received digital subtraction angiography(DSA)and CT diagnosis,and compared the number of true and false cavities of the two methods and the situation of involving iliac artery,mesenteric artery and renal artery,as well as the location of the first break of the aorta and the display of dissection thrombus.Results:Diagnostic test analysis:As the gold standard,the sensitivity and specificity of CT diagnosis were 100.0%(57/57)and 100%(3/3),respectively;There was no significant difference between the two methods in the number of aortic rupture,the position of the first aortic rupture,and the number of true and false lumens(P>0.05);At the same time,there was no significant difference between the two diagnostic methods in involving iliac artery,mesenteric artery and renal artery(P>0.05);The digital subtraction angiography could not display the dissection thrombus.There were 14 cases of dissection thrombus diagnosed by CT,and the display rate of dissection thrombus was 23.33%.The display rate of CT for dissection thrombus was significantly higher than that of digital subtraction angiography(P<0.05).CT images showed double lumen aorta of thoracic aorta,dissection involving left common iliac artery,true and false lumen of dissection,and linear separated transparent shadow was seen between the two lumens in digital subtraction angiography images.Conclusion:In the diagnosis of aortic dissection,CT can obtain higher diagnostic accuracy than digital subtraction angiography,especially in the display of dissection thrombus.
