摘　　要：目的:探究在线尿素清除率监测(OCM)参与监测与评估维持性血液透析充分性的作用。方法:以220例维持性血液透析患者为观察对象,采用OCM监测Kt/V值,测定值≥1.2代表符合充分性标准,统计患者评估结果。结果:首次测定Kt/V值达标率78.18%,达标患者年龄与血流量均大于未达标者;体重与超滤量均小于未达标者,组间对比有差异性。经由血液透析相关参数调整后,原未达标患者达标率75.00%,达标患者年龄小于未达标者,组间对比有差异性。两组患者在体重与血流量及超滤量上对比无明显差异。结论:在维持性血液透析治疗中,OCM的应用可较为精准显示血液透析充分性情况,有助于辅助临床明确血液透析患者不达标原因,进一步作出调整以实现个性化透析治疗。Objective:To explore the role of online urea clearance monitoring(OCM)in monitoring and evaluating the adequacy of maintenance hemodialysis.Methods:220 maintenance hemodialysis patients were taken as observation objects,and Kt/V value was monitored by OCM.The measured value≥1.2 indicated that it met the adequacy standard,and the evaluation results of patients were counted.Results:The rate of reaching the standard of Kt/V value measured for the first time was 78.18%,and the age and blood flow of the patients who reached the standard were higher than those who did not;Body weight and ultrafiltration volume were smaller than those who did not reach the standard,and there was difference between groups.After the adjustment of hemodialysis related parameters,the rate of reaching the standard of the original unqualified patients was 75.00%,and the age of the qualified patients was less than that of the unqualified patients.There was a difference between the groups.There was no significant difference in weight,blood flow and ultrafiltration between the two groups.Conclusion:In the maintenance hemodialysis treatment,the application of OCM can accurately show the adequacy of hemodialysis,which is helpful to assist the clinical diagnosis of the reasons why hemodialysis patients fail to meet the standard,and make further adjustments to realize personalized dialysis treatment.
