摘　　要：目的探索影响泌尿系统疾病诊断相关分组(DRG)合并症与伴随病的其他诊断,为DRG合理优化分组提供参考依据。方法选取2020年1月1日—2022年3月1日病案首页数据,纳入泌尿系统DRG伴有和不伴有合并症与伴随病的病组,采用Logistic分析,先进行单因素分析,纳入有统计学意义的变量,作为模型一;调整年龄、性别和住院时间作为模型二;调整其他有意义的诊断作为模型三。结果共纳入6组DRG病组2223人,年龄和住院天数P<0.05,以不伴有合并症和并发症作为对照组,高血压、糖尿病、肾囊肿、前列腺增生、高脂血症、高尿酸血症、肾功能不全、输尿管狭窄、肾挛缩、脂肪肝、泌尿道感染和贫血的相对危险度(OR)>1。结论采用Logistic探索出泌尿系统疾病合并症与伴随病,能够为制定合理的DRGs分组方案提供有力参考依据。Objective To explore other diagnoses that affect the diagnosis-related grouping(DRG)of urological diseases,comorbidities and concomitant diseases,and provide a reference for the rational optimization of DRG grouping.Methods The data on the front page of medical records from January 1,2020 to March 1,2022 were selected,and the patients with and without comorbidities and concomitant diseases in the urinary system DRG were included.Logistic analysis was used,and univariate analysis was performed first.Variables of scientific significance were used as model 1;age,gender,and length of hospital stay were adjusted as model 2;other meaningful diagnoses were adjusted as model 3.Results A total of 2223 people in 6 DRG disease groups were included,with P<0.05 for age and length of hospital stay.Without comorbidities and complications as the control group,hypertension,diabetes,renal cyst,prostatic hyperplasia,hyperlipidemia,hyperuricemia,renal insufficiency,ureteral stricture,renal contracture,fatty liver,urinary tract infection and anemia with a relative risk(OR)>1.Conclusion For the first time,Logistic was used to explore the complications and concomitant diseases of urinary system diseases,which can provide a strong reference for formulating a reasonable DRGs grouping scheme.
