摘  要:目的为连续性血液净化设备的正确操作提供科学依据,确保重症监护室(ICU)的血液净化质量与患者治疗安全。方法组建专家团队,运用失效模式与效应分析法确定设备故障的失效模式,从严重度、发生频率及探测度三个层面评估,得出事先风险数,探讨降低其数值的维修决策,运用于临床并持续优化。结果实施风险管理后,排序前列的事先风险数值明显下降。结论失效模式与效应分析法能有效降低血滤机的医疗风险,为医护人员操作血滤机,工程师维护管理血滤机保驾护航。Objective To provide scientific basis for the correct operation of continuous blood purification equipment and ensure the quality of blood purification in Intensive Care Unit(ICU)and the safety of patient treatment.Methods An expert team was established to determine the failure mode of equipment failure by using the failure mode and effect analysis method.The severity,frequency and detection were evaluated to obtain the number of risks in advance,discuss the maintenance decision to reduce its value,and apply it to clinical practice and continue to optimize.Results After the implementation of risk management,the prior risk value in the top rank decreased significantly.Conclusion The failure mode and effect analysis method can effectively reduce the medical risk of blood filter,and protect the operation of blood filter by medical staff and the maintenance and management of blood filter by engineers.
