双源CTA联合超声心动图对婴幼儿主动脉弓离断的诊断价值

摘　　要：目的 探讨联合双源CTA(DSCTA)与超声心动图对主动脉弓离断(IAA)的诊断价值。方法 以9例主动脉弓离断患儿为研究对象,分析其超声心动图和DSCTA特征。结果 9例患儿均行超声心动图及DSCTA检查,且均伴有室间隔缺损(VSD),2例动脉导管闭合,2例伴有肺静脉异位引流。超声心动图诊断A型7例,B型1例,1例B型误诊为C型;8例患儿伴有中度以上肺动脉高压(PH)。DSCTA诊断A型7例,B型2例;其中4例伴有PH,但未对肺动脉高压分度。结论 超声心动图联合DSCTA能早期诊断IAA及合并的其他心血管畸形,减少漏误诊,为临床早期手术提供更全面的依据。Objective To evaluate the diagnostic value of combined dual source CTA(DSCTA) and echocardiography in aortic arch rupture.Methods Echocardiography and DSCTA characteristics of 9 children with aortic arch separation were analyzed.Results Echocardiography and DSCTA were performed in all 9 children,all of them were accompanied by ventricular septal defect(VSD),arterio-ductal closure in 2 cases,and ectopic pulmonary vein drainage in 2 cases.Echocardiography diagnosed type A in 7 cases,type B in 1 case,and type B in 1 case was misdiagnosed as type C.Eight patients had moderate or higher pulmonary hypertension(PH).DSCTA diagnosed 7 cases of type A and 2 cases of type B.Four of them had PH,but no hyperbaric index of pulmonary artery.Conclusion Echocardiography combined with DSCTA can early diagnose IAA and other cardiovascular malformations,reduce misdiagnosis and omission,and provide a more comprehensive basis for early clinical operation.
