机采血小板联合冷沉淀输注对颅脑外伤大出血患者凝血功能的影响

摘　　要：目的:分析机采血小板联合冷沉淀输注对颅脑外伤大出血患者凝血功能的影响。方法:选取2018年3月-2020年3月庆阳市人民医院收治的颅脑外伤大出血患者80例为观察对象,采用随机数字表法分对照组与研究组,各40例。对照组给予机采血小板输注治疗,研究组在对照组基础上联合冷沉淀输注治疗。比较两组患者凝血平均时间、输血量、凝血有效率与24 h后凝血功能指标。结果:研究组凝血平均时间与输血量少于对照组,凝血有效率高于对照组,差异有统计学意义(P<0.05);研究组凝血酶时间、凝血酶原时间与部分活化凝血活酶时间水平低于对照组,纤维蛋白原水平高于对照组,差异有统计学意义(P<0.05)。结论:机采血小板联合冷沉淀输注治疗颅脑外伤大出血的患者,能够帮助患者更快地恢复凝血功能,且凝血效果较好。Objective:To analyze investigate the influence of platelet harvesting by machine combined with cryoprecipitate transfusion on coagulation function in patients with massive hemorrhage after craniocerebral trauma.Methods:A total of 80 patients with craniocerebral trauma and massive hemorrhage admitted to Qingyang People's Hospital from March 2018 to March 2020 were selected as observation objects,and they were divided into control group and study group by random number table method,with 40 cases in each group.The control group was treated with machine-harvesting platelet transfusion,and the study group was treated with cryoprecipitate transfusion on the basis of the control group.The average time of coagulation,blood transfunsion volume,the clotting effective rate and coagulation function indexes after 24 hours were compared.Results:The average time of coagulation,blood transfunsion volume in the study group were lower than those in the control group,and the clotting effective rate was higher than that in the control group,with statistically significant difference(P<0.05).The thrombin time,prothrombin time and partially activated thromboplastin time levels in the study group were lower than those in the control group,and the fibrinogen levels were higher than those in the control group,with statistically significant differences(P<0.05).Conclusion:For patients with massive hemorrhage after craniocerebral trauma,the treatment of platelet harvesting by machine combined with cryoprecipitate transfusion can help patients recover coagulation function faster,and the clotting effect is relatively good.
