摘  要:【目的】探讨风湿性心脏病(RHD)合并心房颤动(AF)患者心房组织转化生长因子-β1(TGF-β1)、内皮型一氧化氮合酶(eNOS)、白细胞介素-1β(IL-1β)水平变化及其临床意义。【方法】选择2020年1月至2022年1月两院收治的105例RHD患者,根据是否存在AF分为观察组(合并AF,n=65)和对照组(未合并AF,n=40)。比较两组患者心房组织胶原分布情况和心房组织TGF-β1、eNOS、IL-1βmRNA及蛋白表达水平。【结果】观察组心肌胶原容积分数明显高于对照组(P<0.05)。观察组心房组织TGF-β1、IL-1βmRNA表达水平明显高于对照组(P<0.05),心房组织eNOS mRNA表达水平明显低于对照组(P<0.05)。观察组心房组织TGF-β1、IL-1β蛋白表达水平明显高于对照组(P<0.05),心房组织eNOS蛋白表达水平明显低于对照组(P<0.05)。【结论】TGF-β1、eNOS、IL-1β可能参与了RHD合并AF患者的发病过程,在心肌细胞重构过程中发挥重要作用。【Objective】To investigate the change and its clinical significanc of the levels transforming growth factor-β1(TGF-β1),endothelial nitric oxide synthase(eNOS)and interleukin-1β(IL-1β)in patients with rheumatic heart disease(RHD)complicated with atrial fibrillation(AF).【Methods】A total of 105 patients with RHD admitted to the two hospitals from January 2020 to January 2022 were selected and divided into observation group(with AF,n=65)and control group(without AF,n=40)according to the presence of AF.Distribution of atrial collagen and atrial TGF-β1,eNOS,IL-1β,mRNA and their protein expression levels in two the groups were compared.【Results】The volume fraction of myocardial collagen in the observation group was significantly higher than that in the control group(P<0.05).The expression level TGF-β1 and IL-1βmRNA of atrial tissue in observation group was significantly higher than that of the control group(P<0.05),and the expression level of eNOS mRNA in atrial tissue was significantly lower than that in control group(P<0.05).The protein expression level of TGF-β1 and IL-1βof atrial tissue in the observation group was significantly higher than that of the control group(P<0.05).The expression level of eNOS protein in atrial tissue was significantly lower than that in the control group(P<0.05).【Conclusion】TGF-β1,eNOS and IL-1βmay participate in the pathogenesis of RHD patients complicated with AF,and play an important role in the process of myocardial cell remodeling.
