摘 要:目的 对县(市、区)开展的国家基本公共卫生服务项目绩效评价相关情况进行调查分析,为县(市、区)合理制定绩效评价方案、改进绩效评价方式、减轻基层负担提供决策依据和合理建议。方法 2020年5-6月采用普查和单纯随机抽样相结合的方法,对四川省21个市州、183个县承担基本公共卫生服务项目工作的基层医疗卫生机构(社区卫生服务中心和乡镇卫生院)关键知情人(16 958人)进行问卷和访谈调查,运用Excel 2010和SPSS 22.0软件对数据进行统计分析。对不同地区县级卫生行政、项目管理相关人员调研座谈,对部分基层医疗卫生机构相关工作人员进行半结构式访谈,访谈记录采用主题框架法进行整理分析。结果 问卷调查显示,86.70%的调查对象认为绩效评价对项目工作有促进作用,69.80%的调查对象认为绩效评价方案合理,49.26%的调查对象认为绩效评价工作增加了基层负担,43.41%的人对现有的县级绩效评价工作不满意。不同经济地区对于县级绩效评价相关问题的回答结果差异均明显(χ^(2)=215.94~466.94,P均<0.001)。访谈结果显示,各地均存在现有绩效评价部分指标任务设置不合实际、评价专家水平参差不齐、现场迎检工作繁琐、评价结果运用不足等问题。结论 四川省国家基本公共卫生服务项目县级绩效评价方案设定、组织实施等方面还存在不足,各地应结合本地区实际情况,进一步优化绩效评价方案,充分运用信息化手段,创新评价方式,加强评价结果运用,促进国家基本公共卫生服务高质量发展。Objective To analyze the performance evaluation of national basic public health service projects carried out by counties(cities and districts), thus to provide decision-making basis and reasonable suggestions for counties(cities and districts) to formulate performance evaluation plans,improve performance evaluation methods and reduce the burden. Methods From May to June 2020,16 958 key insiders of grass-roots medical and health institutions(community health service centers and township health centers) in 21 cities and 183 counties were surveyed by questionnaire and interview by using the method of census and random sampling in Sichuan Province. Data were statistically analyzed by excel 2010 and SPSS 22.0 software. Panel discussion was conducted with relevant personnel of health administration and project management at county level in different regions. Furthermore, semi-structured interviews were conducted with relevant staff of some grassroots medical and health institutions. Interview records were collated and analyzed by the method of thematic framework. Results The questionnaire survey shows that 86.70% of the respondents think that the performance evaluation can promote the project work,69.80% of the individuals hold that the performance evaluation scheme is reasonable, 49.26% of the respondents think that the performance evaluation work has increased the burden on the grass-roots level,and 43.41% of the participants were not satisfied with the existing performance evaluation work at the county level.There are significant differences in the answers to questions related to county-level performance evaluation in different economic regions(all P<0.001). The interview results show that there are problems in different aspects, such as unrealistic task setting of some existing performance evaluation indicators, uneven level of evaluation experts, tedious on-site inspection work, and insufficient use of evaluation results.
