摘 要:目的 分析某院骨科临床分离菌中金黄色葡萄球菌的分布以及对常用抗菌药物的耐药性,为骨科合理选用抗菌药物提供依据。方法 收集2015年1月—2020年12月期间该院骨科住院患者分离的金黄色葡萄球菌,对其分布和耐药性进行回顾性分析。结果 共分离出金黄色葡萄球菌608株,其中耐甲氧西林金黄色葡萄球菌(MRSA)163株,占26.81%。金黄色葡萄球菌主要分离自分泌物、坏死组织和尿液,分别占70.56%、16.12%和4.44%。所有金黄色葡萄球菌菌株均未检测出万古霉素、替加环素、利奈唑胺、达托霉素、替考拉宁、头孢洛林耐药株。MRSA对左氧氟沙星和利福平的耐药率<15%,对红霉素、克林霉素耐药率高,分别占94.48%和68.10%。MRSA对克林霉素、利福平、莫西沙星的耐药率显著高于甲氧西林敏感金黄色葡萄球菌。结论 金黄色葡萄球菌对抗菌药物有不同程度的耐药性,应高度重视对其耐药性的监测,合理使用抗菌药物。Objective To analyze the distribution of Staphylococcus aureus in clinical isolates of orthopedics department in a hospital and their resistance to commonly used antimicrobial agents,so as to provide basis for rational selection of antimicrobial agents in orthopedics department.Methods Staphylococcus aureus isolated from orthopedic inpatients in our hospital from January 2015 to December 2020 were collected to retrospectively analyze their distribution and drug resistance.Results A total of 608 strains of Staphylococcus aureus were isolated,including 163 strains of methicillin-resistant Staphylococcus aureus(MRSA),accounting for 26.81%.Staphylococcus aureus was mainly isolated from secretions,necrotic tissues and urine,accounting for 70.56%,16.12% and 4.44%,respectively.No vancomycin,tigecycline,linezolid,daptomycin,teicoplanin and ceftaroline resistant strains were detected in all strains of Staphylococcus aureus.The drug resistance rate of MRSA to levofloxacin and rifampin was less than 15%,and the drug resistance rate to erythromycin and clindamycin was high,accounting for 94.48% and 68.10%,respectively.The drug resistance rate of MRSA to clindamycin,rifampin and moxifloxacin was significantly higher than that of methicillin-sensitive Staphylococcus aureus.Conclusion Staphylococcus aureus has different degrees of resistance to commonly used antimicrobial agents,so we should pay great attention to the monitoring of their drug resistance and rational use of antimicrobial agents.
