补肾填髓汤治疗股骨骨折术后延迟愈合临床研究

摘　　要：目的:观察补肾填髓汤治疗股骨骨折术后延迟愈合的临床疗效及对患者骨形成蛋白-2(BMP-2)、骨钙素(BGP)、碱性磷酸酶(ALP)的影响。方法:选取82例股骨骨折术后延迟愈合患者作为研究对象,按随机数字表法分为对照组与观察组各41例。对照组行常规治疗,观察组在对照组基础上加用补肾填髓汤治疗。比较2组治疗前后Harris髋关节评分、健康调查简表(SF-36)各维度评分及骨代谢指标,并评价2组临床疗效与并发症发生情况。结果:治疗后,观察组总有效率高于对照组(P<0.05),并发症发生率低于对照组(P<0.05)。治疗后,2组Harris髋关节评分、SF-36各维度评分高于治疗前(P<0.05),观察组上述指标高于对照组(P<0.05)。治疗后,2组BMP-2、BGP及ALP水平高于治疗前(P<0.05),观察组骨代谢指标改善优于对照组(P<0.05)。结论:补肾填髓汤治疗股骨骨折术后延迟愈合疗效确切,能有效改善患者骨代谢,提高生活质量,安全性高。Objective:To observe the clinical effect of the therapy of Bushen Tiansui Tang on delayed healing after surgery for femoral fracture and its effect on bone morphogenetic protein-2(BMP-2),osteocalcin(BGP)and alkaline phosphatase(ALP)of patients.Methods:A total of 82 patients with delayed healing after femoral fracture surgery were selected as the research object,and were randomly divided into control group(routine treatment)and observation group(routine treatment+Bushen Tiansui Tang),41 cases in each group.Before and after treatment,the Harris hip scores,each score of 36-Item Short Form Health Survey(SF-36)and indexes of bone metabolism were compared between the two groups.The clinical efficacy and complication rate of the two groups were compared.Results:After treatment,the total effective rate in the observation group was higher than that in the control group(P<0.05),and the incidence of complications in the observation group was lower than that in the control group(P<0.05).After treatment,the Harris hip scores and each score of SF-36 in the two groups were higher than those before treatment(P<0.05),and the above scores in the observation group were higher than those in the control group(P<0.05).After treatment,the levels of BMP-2,BGP and ALP in the two groups were higher than those before treatment(P<0.05),and the levels of BMP-2,BGP and ALP in the observation group were higher than those in the control group(P<0.05).Conclusion:The therapy of Bushen Tiansui Tang has a definite curative effect on delayed healing after surgery for femoral fracture,which can effectively improve the bone metabolism of patients and enhance the quality of life,with high safety profile.
