盐酸多奈哌齐联合奥氮平对血管性痴呆患者精神行为障碍、脑血流及神经营养因子水平的影响

摘　　要：目的分析脑梗塞后血管性痴呆患者使用盐酸多奈哌齐联合奥氮平的干预效果。方法选取我科2019年1月~2021年2月期间的127例脑梗塞后血管性痴呆患者,按照抽签方式分为两组。对照组63例给予奥氮平治疗,观察组64例联合盐酸多奈哌齐治疗,对比两组患者精神行为障碍、脑血流指标、相关血液因子指标及安全性。结果干预后,观察组中动脉(MCA)、后动脉(PCA)、椎动脉(VA)血流速度、神经营养因子3(NT3)及脑源性神经营养因子(BDNF)水平高于对照组(P<0.05);观察组血清C反应蛋白(CRP)、血清β-淀粉样蛋白(Aβ)水平、痴呆病理行为评定量表(BEHAVE-AD)评分、简易智力状态检查量表(MMSE)评分较对照组低(P<0.05);两组患者不良反应无统计学意义(P>0.05)。结论脑梗塞后血管性痴呆患者使用盐酸多奈哌齐联合奥氮平治疗能够有效改善患者精神行为障碍及相关血液因子指标,提高脑血流量,安全性高。Objective To analyze the intervention effect of donepezil hydrochloride combined with olanzapine in patients with vascular dementia after cerebral infarction.Methods From January 2019 to February 2021,127 patients with vascular dementia after cerebral infarction in our department were divided into two groups according to the lottery method.63 patients in the control group were treated with olanzapine,and 64 patients in the observation group were treated with donepezil hydrochloride.The mental behavior disorders,cerebral blood flow indexes,and related blood factor indexes were compared between the two groups.Results After intervention,the blood flow velocity of middle artery(MCA),posterior artery(PCA),vertebral artery(VA),neurotrophic factor 3(NT3)and brain-derived neurotrophic factor(BDNF)levels in the observation group were higher than those in the control group(P<0.05);the observation group’s serum C-reactive protein(CRP),serumβ-amyloid(Aβ)levels,dementia pathological behavior assessment scale(BEHAVE-AD)score,and simple mental status check scale(MMSE)score were lower than those of the control group(P<0.05).The adverse reactions of the two groups of patients were not statistically different(P>0.05).Conclusion The treatment of donepezil hydrochloride combined with olanzapine in patients with vascular dementia after cerebral infarction can effectively improve their mental behavior disorder and related blood factor indexes,and increase cerebral blood flow.
