摘 要:目的:研究组织多普勒速度成像技术结合经胸实时三维超声心动图在缺血性二尖瓣反流(ischemic mitral regurgitatio,IMR)患者二尖瓣构型中的评估价值。方法:选取我院于2018年6月~2020年12月收治的140例IMR患者,将同期100例体检健康者作为对照组,均行组织多普勒速度成像技术、经胸实时三维超声心动图检测。比较两组受试者的各影像学参数,采用Pearson相关性分析各参数与Gensini积分的相关性,采用多元Logistic回归分析发生IMR的影响因素。结果:研究组的组织多普勒速度成像各参数(SPV、SPDC、SPAC、SPDT、SPAT、SRT)均高于对照组(P<0.05)。研究组患者的三维二尖瓣结构参数(DAP、ALPM、AC、A3DT、NPA、Htent、Vtent)均高于对照组(P<0.05)。Pearson相关性分析显示,DAP、AC、A3DT、Htent、Vtent以及SPDC、SPAC、SPDT、SPAT、SRT与Gensini积分呈正相关性(P<0.05),ALPM、NPA以及SPV与Gensini积分无相关性(P>0.05)。多元Logistic回归分析显示,SPDT、SPAT、SRT、A3D、AC、Htent、Vtent增加,则发生IMR的风险更大(P<0.05)。结论:组织多普勒速度成像技术以及经胸实时三维超声心动图可有效评价IMR患者的乳头肌的结构、功能,各参数与冠状动脉狭窄程度存在相关性,可作为IMR发生的影响因素。140 patients with IMR treated in our hospital from June 2018 to December 2020 were selected,and 100 healthy persons in the same period were taken as the control group.All of them were detected by tissue Doppler velocity imaging and transthoracic real-time three-dimensional echocardiography.The imaging parameters of the two groups were compared,the correlation between the parameters and Gensini score was analyzed by Pearson correlation,and the influencing factors of IMR were analyzed by multiple logistic regression.The tissue Doppler velocity imaging parameters(SPV,SPDC,SPAC,SPDT,SPAT,SRT)of the study group were higher than those of the control group(P<0.05).The three-dimensional mitral valve structure parameters(DAP,ALPM,AC,A3DT,NPA,Htent,Vtent)of the study group were higher than those of the control group(P<0.05).Pearson correlation analysis showed that DAP,AC,A3DT,Htent,Vtent,SPDC,SPAC,SPDT,SPAT,SRT were positively correlated with Gensini points(P<0.05),and ALPM,NPA,and SPV were not correlated with Gensini points(P>0.05).Multivariate logistic regression analysis showed that SPDT,SPAT,SRT,A3D,AC,Htent,Vtent increased,the risk of IMR was greater(P<0.05).Tissue Doppler velocity imaging technology and transthoracic real-time three-dimensional echocardiography can effectively evaluate the structure and function of the papillary muscles of patients with IMR.There is a correlation between various parameters and the degree of coronary artery stenosis,which can be used as influencing factors for the occurrence of IMR.
