急性心肌梗死患者外周血lncRNA GAS5水平与左心室结构、功能的关系分析

摘　　要：目的探讨急性心肌梗死患者外周血长链非编码RNA(lncRNA)生长停滞特异性转录本5(GAS5)(lncRNA GAS5)水平与左心室结构、功能的相关性。方法前瞻性选取2017年2月至2019年3月北京市大兴区人民医院收治的急性心肌梗死拟行经冠状动脉介入术(PCI)术治疗的149例患者,将其设为疾病组;另选取同期体检健康者91例,将其设为健康组。两组均行外周血lncRNA GAS5、lncRNA牛磺酸上调基因1(lncRNA TUG1)、B型利钠肽(BNP)、肌酸激酶同工酶(CK-MB)、心肌肌钙蛋白T(cTnT)水平及左心室结构、功能相关指标[左心室舒张末期内径(LVEDd)、左心室收缩末内径(LVESD)、左心室后壁厚度(LVPWTD)、左心室舒张末期室间隔厚度(LVSTD)、左心室舒张末期容积(LVEDV)、左心室收缩末期容积(LVESV)、左心室射血分数(LVEF)、心排血量(CO)]检测。另对比左心室重构组与非左心室重构组各指标水平以及lncRNA GAS5、左心室结构与功能术前术后的变化率,并分析lncRNA GAS5水平与各指标的相关性,且分析lncRNA GAS5水平变化率与左心室结构、功能指标变化率的关系,另评估lncRNA GAS5水平变化率对左心室重构的预测价值。结果疾病组入院后即刻的lncRNA GAS5、lncRNA TUG1、BNP、CK-MB、cTnT水平及LVEDd、LVESD、LVPWTD、LVSTD、LVEDV、LVESV均高于疾病组术后6个月及健康组,LVEF、CO水平均低于疾病组术后6个月及健康组,差异均有统计学意义(P<0.05)。疾病组发生左心室重构者56例,占37.58%(56/149)。左心室重构组入院后即刻及术后6个月的lncRNA GAS5、lncRNA TUG1、BNP、CK-MB、cTnT及LVEDd、LVESD、LVPWTD、LVSTD、LVEDV、LVESV水平均明显高于非左心室重构组,LVEF、CO均低于非左心室重构组,差异均有统计学意义(P<0.05)。经Pearson相关性分析,疾病组lncRNA GAS5水平与LVEF、CO呈负相关性(P<0.05),与lncRNA TUG1、BNP、CK-MB、cTnT及LVEDd、LVESD、LVPWTD、LVSTD、LVEDV、LVESV均呈正相关性(P<0.05)。左心Objective To explore the relationship between the level of long-chain non-coding RNA(lncRNA)growth arrest specific transcript 5(GAS5)(lncRNA GAS5)and and the structure and function of left ventricle in patients with acute myocardial infarction.Methods A total of 149 patients with acute myocardial infarction who were treated by percutaneous coronary intervention(PCI)in Beijing Daxing District People's Hospital from February 2017 to March 2019 were prospectively selected as the disease group;physical examinations during the same period were also selected.91 healthy subjects were selected as the healthy group.The levels of lncRNA GAS5,lncRNA taurine up regulated gene 1(lncRNA TUG1),B-type natriuretic peptide(BNP),creatine kinase isoenzyme(CK-MB),cardiac troponin T(cTnT)in peripheral blood and left ventricular structure and function related indexes[left ventricular end diastolic diameter(LVEDd),left ventricular end systolic diameter(LVESD),left ventricular posterior wall thickness(LVPWTD),left ventricular end diastolic septal thickness(LVSTD),left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume(LVESV),left ventricular ejection fraction(LVEF),cardiac output(CO)]were detected of the two groups.The each index of left ventricular reconstruction group and non left ventricular reconstruction group and the changes rate of lncRNA GAS5 and left ventricular structure and function indexes were compared,and the relationship between the level of lncRNA GAS5 and each index were analyzed,and the relationship between the change rate of lncRNA GAS5 level and the change rates of left ventricular structure and function indexes were analyzed,then the predictive value of the rate of change of lncRNA GAS5 level on left ventricular remodeling was evaluated.Results The lncRNA GAS5,lncRNA TUG1,BNP,CK-MB,cTnT,LVEDd,LVESD,LVPWTD,LVSTD,LVEDV and LVESV in the disease group at immediately after admission were higher than those in the disease group at 6 months after operation and in the healthy group,and the LVEF and C
