摘  要:目的通过研究高浓度氢氧气治疗爆震性耳聋的效果,为临床治疗提供参考。方法取健康清洁级、ABR阈值正常的豚鼠15只,雌雄不限,体重为250-300g。将其随机分成3组,空白对照组、噪声组、氢氧气治疗组(于噪声暴露后1小时给予高浓度氢氧气吸入治疗)。给予脉冲噪声(压力峰值为163 dB SPL,脉宽为0.25ms)暴露30次。于噪声暴露前、暴露后即刻及氢氧气治疗10次后测听性脑干反应(ABR)。治疗结束后通过对耳蜗毛细胞染色观察其形态。结果与噪声组相比,高浓度氢氧气治疗可以降低豚鼠4kHz的ABR阈值,有统计学意义(P=0.022<0.05)。氢氧气治疗组外毛细胞纤毛的缺失、倒伏等现象较噪声组少。结论高浓度氢氧气在治疗爆震性耳聋方面有一定的效果。Objective To report the efficacy of high-concentration hydrogen and oxygen in the treatment of blastinduced hearing loss.Methods Fifteen healthy guinea pigs with normal ABR thresholds were(male or female,250-300 g)were randomly divided into a control group,a noise exposure only group,and a hydrogen and oxygen treatment group(high-concentration hydrogen and oxygen inhalation 1 hour after noise exposure).Exposure includes 30 rounds of impulse noise(peak pressure=163 dB SPL,pulse width=0.25 ms).The audiometric brainstem responses(ABR)were measured before and immediately after exposure,and after hydrogen and oxygen therapy(ten times)with examination of morphology of cochlear hair cells.Results Compared with the noise exposure only group,animals receiving highconcentration hydrogen and oxygen therapy showed less ABR threshold elevation at 4 kHz(P=0.022<0.05),with less loss and flattening of outer hair cells.Conclusion High-concentration of hydrogen and oxygen has certain therapeutic effects for blast-induced hearing loss.
