妊娠中期孕妇血清游离β-人绒毛膜促性腺激素与前置胎盘合并胎盘植入的关联性

摘　　要：目的探讨妊娠中期游离β-人绒毛膜促性腺激素(free-β-HCG)与胎盘植入之间的关系。方法选自2016年4月至2020年7月于济宁医学院附属医院终止妊娠的孕妇504例,其中前置胎盘合并胎盘植入者即为胎盘植入组105例,单纯前置胎盘者为前置胎盘组122例,健康孕妇277例即对照组。回顾性分析其临床资料,收集血清筛查中free-β-HCG值,并进行统计学分析。结果胎盘植入组free-β-HCG水平高于前置胎盘组和对照组(P<0.001)。在调整取样时的孕周、体重指数和年龄后,血清free-β-HCG与胎盘植入有关联性(P<0.001)。结论妊娠中期血清free-β-HCG水平升高与胎盘植入存在关联性,提示free-β-HCG可能在预测前置胎盘合并胎盘植入中发挥作用。Objective To investigate the relationship between Free-β-HCG and placenta accreta in the second trimester of pregnancy.Methods A total of 504 pregnant women from April 2016 to July 2020 were enrolled in this study(105 patients with placenta accreta,122 patients with non-adherent placenta previa,and 277 healthy pregnant controls)。Their clinical data were analyzed retrospectively,and the free-β-HCG values of serological screening in second-trimester pregnancy were collected and statistically analyzed.Results There was no statistical significance in maternal age,BMI,reproductive history,abortion history,cesarean section history,gestational age of blood collection and HCG value.The serum free-β-HCGlevel of the placentaaccreta group is higher than that of the placenta previa and the control group(P<0.001).Serum free-β-HCG was correlated with placenta accreta after adjusting for gestational week at the time of blood sampling,body mass index,and age(P<0.001).Conclusion Increased level of second trimester serum free-β-HCG was associated with placenta accreta,whichindicatesthe possible role of free-β-HCG in the prediction of placenta previa complicated with placental accreta.
