利湿通瘀方湿敷治疗下肢静脉曲张伴淤积性皮炎临床观察

摘　　要：目的探讨利湿通瘀方湿敷联合综合干预应用于下肢静脉曲张伴淤积性皮炎对患者血液流变学指标、血清炎性因子水平、中医证候的影响。方法本研究对象均从翼城县中医医院2018年1月—2021年1月收治的60例下肢静脉曲张伴淤积性皮炎患者中选取,分为对照组和观察组,各30例(随机数字表法)。对照组给予曲张静脉抽剥术、综合干预和术后常规治疗,观察组在对照组的基础上给予利湿通瘀方湿敷,均于治疗后随访1个月。比较2组手术情况、治疗前后中医证候积分、血液流变学指标、血清炎性因子水平变化。结果2组相比,患者手术时间、术中出血量差异无统计学意义(P>0.05);与对照组比,观察组患者创面愈合时间、术后住院时间均缩短,住院费用减少;与治疗前比较,治疗后2组患者各项证候积分、总积分、全血黏度(高切)、全血黏度(低切)、血浆黏度和血清IL-8、IL-17、TNF-α水平均降低,且观察组低于对照组(P<0.05)。结论下肢静脉曲张伴淤积性皮炎应用综合干预联合利湿通瘀方湿敷能有效减轻患者临床症状,改善机体血液流变学,减轻炎症反应,促进创面愈合。Objective To investigate the effect of removing dampness and removing blood stasis Chinese herbal medicine hydropathic compress combined with comprehensive intervention applied to lower extremity varicose veins with stasis dermatitis on hemorheology indexes,serum inflammatory factor levels,and traditional Chinese medicine(TCM)syndromes.Methods The study subjects were selected from 60 patients with varicose veins and stasis dermatitis in Yicheng Hospital of Traditional Chinese Medicine from January 2018 to January 2021.They were divided into control group and observation group,with 30 cases in each group,by random number table method.The control group was given varicose vein stripping,comprehensive intervention and postoperative routine treatment.The observation group was given removing dampness and removing blood stasis Chinese herbal medicine hydropathic compress on the basis of the control group.All were followed up for 1 month after treatment.The operation conditions,TCM syndrome integrals,hemorheology indexes,and serum inflammatory factor levels were compared between the two groups before and after treatment.Results Compared with the two groups,there was no significant difference in operation time and intraoperative blood loss(P>0.05).Compared with the control group,the observation group had shorter wound healing time and postoperative hospital stay,and hospitalization expenses were reduced;compared with before treatment After treatment,the scores of various syndromes,total scores,whole blood viscosity(high-cut),whole blood viscosity(low-cut),plasma viscosity and serum IL-8,IL-17,and TNF-α levels decreased in the two groups.And the observation group was lower than the control group(P<0.05).Conclusion The comprehensive intervention of lower extremity varicose veins with stasis dermatitis can effectively relieve the clinical symptoms of patients,improve the body’s blood rheology,reduce inflammation,and promote wound healing.
