摘 要:目的 探讨围绝经期女性雌激素水平变化与冠心病发病的相关性。方法 63例围绝经期女性,通过冠状动脉造影检查结果分成实验组(冠心病, 30例)和参考组(非冠心病, 33例)。两组均进行实验室指标检查、冠状动脉造影检查。比较两组雌二醇(E2)和血清生化指标[总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、同型半胱氨酸(Hcy)、N末端脑钠肽前体(NT-pro BNP)、低密度脂蛋白胆固醇(LDL-C)]水平,分析实验组冠状动脉病变程度与NT-pro BNP、E2水平的相关性。结果 实验组的E2(12.86±4.91)pg/ml低于参考组的(20.06±7.18)pg/ml, LDL-C(3.29±1.21)mmol/L、TC(4.92±1.23)mmol/L、NT-pro BNP(328.9±187.2)pg/ml均高于参考组的(2.59±0.76)mmol/L、(3.96±1.07)mmol/L、(149.9±117.6)pg/ml,差异均有统计学意义(P<0.05);实验组HDL-C略低于参考组,Hcy、TG略高于参考组,但差异无统计学意义(P>0.05)。轻度冠状动脉病变患者NT-pro BNP水平为(201.5±95.8)pg/ml, E2水平为(14.64±4.58)pg/ml;中度冠状动脉病变患者NT-pro BNP水平为(317.5±52.4)pg/ml, E2水平为(12.25±1.28)pg/ml;重度冠状动脉病变患者NT-pro BNP水平为(581.9±172.9)pg/ml,E2水平为(10.41±1.71)pg/ml。重度冠状动脉病变患者的血清NT-pro BNP水平高于轻、中度冠状动脉病变患者, E2水平低于轻、中度冠状动脉病变患者,差异具有统计学意义(P<0.05)。Pearson相关性分析结果显示,冠状动脉病变程度与NT-pro BNP水平呈正相关(r=0.542, P<0.05),与E2水平呈负相关(r=-0.368,P<0.05)。结论 围绝经期女性雌激素水平变化和冠心病的发生及病变程度具有一定相关性。Objective To discuss the correlation between the change of estrogen level and coronary heart disease in perimenopausal women. Methods A total of 63 perimenopausal women were divided into experimental group(coronary heart disease, 30 cases) and reference group(non-coronary heart disease, 33 cases) by coronary angiography findings. Laboratory tests and coronary angiography were performed in both groups. Comparison was made on estradiol(E2) and serum biochemical indicators [total cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C), homocysteine(Hcy), N-terminal-pro-brain natriuretic peptide(NT-pro BNP)and low-density lipoprotein cholesterol(LDL-C)] between the two groups, and the correlation between severity of coronary artery disease and NT-pro BNP and E2 in the experimental group was analyzed. Results The E2(12.86±4.91) pg/ml in the experimental group was lower than(20.06±7.18) pg/ml in the reference group;the LDL-C(3.29±1.21) mmol/L, TC(4.92±1.23) mmol/L, NT-pro BNP(328.9±187.2) pg/ml in the experimental group were higher than(2.59±0.76) mmol/L,(3.96±1.07) mmol/L,(149.9±117.6) pg/ml in the reference group;the differences were all statistically significant(P<0.05). The HDL-C in the experimental group was slightly lower than that in the reference group, and the Hcy and TG were slightly higher than those in the reference group, but the difference was not statistically significant(P>0.05). In patients with mild coronary artery lesions, the NT-pro BNP levels were(201.5±95.8) pg/ml and the E2 levels were(14.64±4.58) pg/ml;in patients with moderate coronary artery lesions, the NT-pro BNP level was(317.5±52.4) pg/ml and the E2 level was(12.25±1.28) pg/ml;in patients with severe coronary artery disease, the NT-pro BNP level was(581.9±172.9) pg/ml and the E2 level was(10.41±1.71) pg/ml.
