经阴茎腹侧松解固定成形术治疗小儿隐匿阴茎的疗效分析

摘　　要：目的探讨经阴茎腹侧松解固定成形术治疗小儿隐匿阴茎的临床疗效。方法84例隐匿阴茎患儿,根据手术方法不同分为观察组与对照组,各42例。观察组行经阴茎腹侧松解固定成形术,对照组行Brisson术。对比两组临床疗效、并发症发生情况、患儿监护人对阴茎外观满意度。结果观察组治疗总有效率95.24%高于对照组的80.95%,差异有统计学意义(P<0.05)。观察组并发症发生率7.14%低于对照组的23.81%,差异有统计学意义(P<0.05)。观察组患儿监护人对阴茎整体外观、阴茎长度、勃起时阴茎伸直状态、阴茎头外形、尿道口位置和形状、包皮分布评分分别为(2.83±0.56)、(2.78±0.30)、(2.88±0.25)、(2.80±0.24)、(2.73±0.54)、(2.79±0.37)分,均高于对照组的(2.13±0.36)、(2.40±0.29)、(2.42±0.29)、(2.38±0.22)、(2.37±0.35)、(2.29±0.36)分,差异有统计学意义(P<0.05)。结论经阴茎腹侧松解固定成形术是治疗小儿隐匿阴茎的理想术式,操作简便,外观理想,能够显著增强疗效,提高手术安全性和患儿监护人对阴茎外观的满意程度。Objective To discuss the clinical efficacy of ventral penile release and fixation plasty in the treatment of concealed penis in children.Methods A total of 84 children with concealed penis were divided into observation group and control group according to different surgical methods,with 42 cases in each group.The observation group was treated with ventral penile release and fixation plasty,and the control group was treated with Brisson's surgery.The clinical efficacy,the incidence of complications,and the satisfaction of the guardians of the children with the appearance of the penis were compared between the two groups.Results The total effective rate 95.24%of the observation group was higher than 80.95%of the control group,and the difference was statistically significant(P<0.05).The incidence of complications 7.14%in the observation group was lower than 23.81%in the control group,and the difference was statistically significant(P<0.05).The scores of overall penile appearance,penile length,penile extension during erection,glans penis shape,position and shape of the urethra,and foreskin distribution by the guardians of the children in the observation group were(2.83±0.56),(2.78±0.30),(2.88±0.25),(2.80±0.24),(2.73±0.54),and(2.79±0.37)points,which were higher than(2.13±0.36),(2.40±0.29),(2.42±0.29),(2.38±0.22),(2.37±0.35),and(2.29±0.36)points in the control group,and the differences were all statistically significant(P<0.05).Conclusion Ventral penile release and fixation plasty is an ideal surgery for the treatment of concealed penis in children.It is easy to operate and has an ideal appearance,which can significantly enhance the efficacy,improve the safety of the surgery and the satisfaction of the child's guardian with the appearance of the penis.
