摘  要:目的探讨18氟脱氧葡萄糖发射型正电子断层-计算机断层显像扫描(F-FDG PET/CT)体积代谢参数对晚期宫颈癌同步放化疗效果的预测价值。方法选取徐州市中心医院2018年6月至2020年11月收治的122例晚期宫颈癌患者作为研究对象,均行同步放化疗,且治疗前均接受F-FDG PET/CT检查。根据同步放化疗效果,分为有效组和无效组,对比两组F-FDG PET/CT体积代谢参数,并采用受试者工作特征(ROC)曲线分析其对同步放化疗效果的预测价值。结果同步放化疗无效率为41.53%、有效率为58.47%;无效组患者的F-FDG PET/CT体积代谢参数最大标准摄取值(SUV)、代谢体积(MTV)、病灶糖酵解总量(TLG)均高于有效组患者(P<0.05);SUV、MTV、TLG联合预测同步放化疗无效的灵敏度、特异度、曲线下面积(AUC)分别为97.96%、91.30%、0.914,联合预测的灵敏度、AUC均高于单独预测(P<0.05),特异度与单独预测比较差异无统计学意义(P>0.05)。结论F-FDG PET/CT体积代谢参数可预测晚期宫颈癌同步放化疗效果,SUV、MTV、TLG联合预测的效能更高。Objective To investigate the predictive value of volume metabolism parameters of 18 fluorodeoxyglucose emission positron tomography-computed tomography(F-FDG PET/CT)in concurrent chemoradiotherapy for advanced cervical cancer.Methods A total of 122 patients with advanced cervical cancer admitted to Xuzhou Central Hospital from June 2018 to November 2020 were selected as research objects.All patients were intended to receive concurrent chemoradiotherapy and receivedF-FDG PET/CT examination before treatment.According to the effect of concurrent chemoradiotherapy,the patients were divided into the effective group and the ineffective group.TheF-FDG PET/CT volume metabolism parameters of the two groups were compared,and the receiver operating characteristic(ROC)curve was used to analyze its predictive value for concurrent chemoradiotherapy.Results The ineffective rate of concurrent chemoradiotherapy was 41.53%,and the effective rate was 58.47%.The maximum standard uptake of metabolic parameters(SUV),metabolic volume(MTV)and total glycolysis(TLG)ofF-FDG PET/CT volume in ineffective group were higher than those in effective group(P<0.05).The sensitivity,specificity and AUC of SUV,MTV and TLG in predicting in-effective concurrent chemoradiotherapy were 97.96%,91.30% and 0.914 respectively,and the sensitivity and AUC of combined prediction were higher than those of single prediction(P<0.05),and there was no significant difference in the specificity between the specificity and single prediction(P>0.05).Conclusions TheF-FDG PET/CT volume metabolic parameters can predict the effect of concurrent chemoradiotherapy for advanced cervical cancer,and the combined prediction of SUV,MTV and TLG is more effective.
