摘　　要：目的探讨不同临床分期的梅毒患者血清甲苯胺红不加热血清试验(TRUST)滴度与C反应蛋白(CRP)水平的相关性。方法选取2018年1月至2021年12月北京市大兴区人民医院诊治的227例梅毒患者作为研究对象,对其临床资料进行回顾性分析。根据患者的梅毒临床分期分为A组(Ⅰ期梅毒,122例)、B组(Ⅱ期梅毒,74例)、C组(Ⅲ期梅毒,31例)。比较三组的血清TRUST滴度和CRP。分析血清TRUST滴度和CRP对梅毒Ⅱ期与非Ⅱ期(Ⅰ期+Ⅲ期)患者的鉴别诊断价值。结果三组血清TRUST滴度和CRP比较,差异具有统计学意义(F=37.536、46.687,P<0.05)。B组血清TRUST滴度及CRP水平均高于A组和C组,差异具有统计学意义(P<0.05);A组、C组血清TRUST滴度及CRP水平比较,差异无统计学意义(P>0.05)。采用血清TRUST滴度和CRP对梅毒患者的分期进行诊断,最佳截断值分别为10.25、50.34mg/L,约登指数为45.21%、34.31%。结论不同临床分期梅毒患者的血清TRUST滴度和CRP水平具有明显差异。Ⅱ期梅毒患者的血清TRUST滴度和CRP水平最高,Ⅰ期和Ⅲ期梅毒患者的血清TRUST滴度、CRP水平接近。Objective To explore the correlation between serum toluidine red unheated serum test(TRUST)titer and C reactive protein(CRP)levels in patients in different clinical stages of syphilis.Methods A retrospective analysis of the data of 227 patients diagnosed with syphilis in the Daxing District People's Hospital from January 2018 to December 2021 was selected.According to the clinical stages of syphilis,the patients were divided into group A(stage Ⅰ syphilis,122 cases),group B(stage Ⅱ syphilis,74 cases),and group C(stage Ⅲ syphilis,31 cases).Serum TRUST titer and CRP were compared among the three groups.The value of serum TRUST titer and CRP in the differential diagnosis of stage Ⅱ and non-stage Ⅱ syphilis(stage Ⅰ+stage Ⅲ)was analyzed.Results There were significant differences in serum TRUST titer and CRP among the three groups(F=37.536,46.687,P<0.05).The serum TRUST titer and CRP levels in group B were higher than those in groups A and group C(P<0.05).There was no significant difference in serum TRUST titer and CRP levels between group A and group C(P>0.05).The optimal cut-off values were 10.25 and 50.34 mg/L,respectively,and the Youden index was 45.21% and 34.31%.Conclusions There were significant differences in serum TRUST titer and CRP levels in syphilis patients with different clinical stages.The serum TRUST titer and CRP level of stage Ⅱ syphilis patients were the highest,and the serum TRUST titer and CRP level of stage Ⅰ and Ⅲ syphilis patients were close.
