健脾固肾化瘀汤对糖尿病肾脏病大鼠肾脏保护及IGF-1 mRNA、MCp-1 mRNA和Nephrin mRNA表达的影响

摘　　要：目的:研究健脾固肾化瘀汤对糖尿病肾脏病(DKD)大鼠肾脏病理变化的改善及肾病蛋白(Nephrin)、单核细胞趋化因子1(MCp-1)和类胰岛素1号生长因子(IGF-1)的影响。方法:采用雄性Sprague-Dawley大鼠腹腔注射链脲霉素(STZ)来复制糖尿病肾病大鼠模型。根据随机数表随机选择6组,即正常组、模型组、雷米普利组、健脾固肾化瘀汤低、中、高剂量组(1.9、3.8、7.6 g/kg),各10只。用药8周后,测定各组血清肌酐、尿素氮、甘油三酯、总胆固醇和24 h尿白蛋白的指标。在光学显微镜下观察肾脏的形态学变化。用免疫组化法观察Nephrin、MCp-1和IGF-1的表达,用RT-PCR方法观察Nephrin、MCp-1、IGF-1 mRNA的表达。结果:与正常组相比,模型组的甘油三酯、总胆固醇、肌酐、尿素氮、24 h尿微量白蛋白、MCp-1和IGF-1 mRNA的水平明显升高(P<0.05),Nephrin mRNA的表达明显降低(P<0.05);与模型组相比,健脾固肾化瘀汤各组的甘油三酯、总胆固醇、肌酐、尿素氮、24 h尿微量白蛋白、MCp-1、IGF-1 mRNA水平下降(P<0.05),Nephrin mRNA水平明显提高(P<0.05),且大鼠肾脏组织细胞排列整齐。结论:健脾固肾化瘀汤可下调IGF-1和MCp-1 mRNA的表达,上调Nephrin mRNA的表达,改善肾组织的病理变化,起到保护作用。Objective:To study the effect of Jianpi Gushen Huayu Decoction on the renal pathological changes of diabetes nephropathy(DKD) rats and the influence of nephrotic protein(Nephrin),monocyte chemoattractant protein-1(MCp-1) and insulin-like growth factor 1(IGF-1).Methods:Male Sprague-Dawley rats were intraperitoneally injected with streptozotocin(STZ) to replicate the model of diabetes nephropathy.According to the random number table,6 groups were randomly selected,namely normal group,model group,Ramipril group,low-,middle-and high-dose groups of Jianpi Gushen Huayu Decoction(1.9,3.8,7.6 g/kg),with 10 rats in each group.After 8 weeks of administration,the indexes of serum creatinine(Cr),blood urea nitrogen(BUN),triglyceride(TG),total cholesterol(TC) and 24 h urinary albumin(ALB) in each group were measured.The morphological changes of kidney were observed under light microscope.The expressions of Nephrin,MCp-1 and IGF-1 were observed by immunohistochemistry,and the expressions of Nephrin,MCp-1 and IGF-1 mRNA were observed by RT-PCR.Results:Compared with the normal group,the levels of TG,TC,Cr,BUN,24 h ALB,MCp-1 and IGF-1 mRNA in the model group were significantly increased(P<0.05),and the expression of Nephrin mRNA was significantly decreased(P<0.05);Compared with the model group,the levels of TG,TC,Cr,BUN,24 h ALB,MCp-1 and IGF-1 mRNA in each group of Jianpi Gushen Huayu Decoction were decreased(P<0.05),while Nephrin mRNA was significantly increased(P<0.05),and the kidney cells were arranged in order.Conclusion:Jianpi Gushen Huayu Decoction can down regulate the expression of IGF-1 and MCp-1 mRNA,up regulate the expression of Nephrin mRNA,improve the pathological changes of kidney tissue and play a protective role.
