摘 要:目的探讨存在颈椎后凸畸形的脊髓型颈椎病患者的磁共振特征,为临床实施更精准的治疗提供影像学依据。方法选取222例脊髓型颈椎病患者的磁共振图像,利用矢状位T_(2)WI图像将患者分为后凸组及曲度正常组,对两组患者的年龄、性别、椎管的前后径、椎管的横截面积及T_(2)信号强度进行统计学分析。结果后凸组患者60例,平均年龄(50.6±7.9)岁,曲度正常组患者162例,平均年龄(57.5±8.9)岁,两组患者的年龄差异有统计学意义(t=4.19,P<0.01),两组患者的椎管前后径及椎管的横截面积差异具有统计学意义(t=7.71,P<0.01;t=8.24,P<0.01),两组患者脊髓内T_(2)高信号的信号强度差异具有统计学意义(χ^(2)=5.18,P<0.05)。结论后凸组脊髓型颈椎病患者较正常曲度组患者更年轻,存在较大的椎管前后径及椎管横截面积,脊髓内可出现更高强度的T_(2)信号,这些影像学特征为临床评估后凸畸形患者的病情和制定更精准的治疗方案提供了依据。Objective The objective of the study was to analyse the MRI features of cervical spondylotic myelopathy patients with kyphosis in order to provide accurate imaging basis for clinical treatment.Methods The magnetic resonance images of 222 patients with cervical spondylotic myelopathy were retrospectively analyzed.The sagittal T_(2)WI images were used to divide the patients into kyphosis group and normal curvature group.The age,sex,anterior and posterior diameter of spinal canal,cross-sectional area of spinal canal and T_(2) signal intensity were statistically analyzed.Results The average age of 60 patients in kyphosis group was(50.6±7.9)years old,with 162 patients in normal curvature group,the average age was(57.5±8.9)years old,the two groups were statistically significant(t=4.19,P<0.01).The difference of anterior and posterior diameter and cross-sectional area of spinal canal between the two groups was statistically significant(t=7.71,P<0.01;t=8.24,P<0.01),and the difference of signal intensity of high signal in spinal between the two groups was statistically significant(χ^(2)=5.18,P<0.05).Conclusion Compared with the normal curvature group,the patients with cervical spondylotic myelopathy in the kyphosis group are younger,have larger anterior and posterior diameters of the spinal canal and cross-sectional area of the spinal canal,higher intensity of spinal T_(2) signals appeared.These imaging features provide a basis for evaluation the severity of cervical spondylotic myelopathy patients with kyphosis,which is helpfue for accurate treatment.
