关键节点控制在经PICC化疗的泌尿系统肿瘤患者中应用效果的研究

摘　　要：目的探讨关键节点控制对于提升经外周静脉置入中心静脉导管(PICC)化疗的泌尿系统肿瘤患者护理质量的效果。方法确定经PICC化疗患者质量控制的3个关键节点,分别为PICC的维护、减轻化疗相关不良反应以及预防医院获得性感染的发生,并选取158例患者实施基于关键节点控制的护理方案,以评价该方案的有效性。结果基于关键节点控制的护理方案可以有效降低PICC化疗患者化疗期间穿刺处局部感染(P=0.013)、导管脱出(P<0.001)、导管堵塞(P=0.013)、化疗相关不良反应(P<0.001)和导管相关性血流感染(P=0.013)的发生率。结论关键节点控制对经PICC化疗的泌尿系统肿瘤患者起到了积极作用,提示护理人员需要对静脉化疗患者的PICC管路进行规范化管理、关注患者化疗过程中的不良反应,并实施精准护理,从而提高护理质量和护理效率。Objective To explore clinical effects of critical control point in urological cancer patients carrying PICC catheter for chemotherapy.Methods Three critical control points were determined in patients carrying PICC catheter for chemotherapy:PICC maintenance,decrease in chemotherapy related adverse events and prevention of hospital-acquired infection.We selected 158 patients in the study and evaluated the clinical effects.Results The total incidence of PICC complications in patients was lower than before,which including PICC infections(P=0.013),catheter prolapse(P<0.001),blockage(P=0.013),chemotherapy related complications(P<0.001)and catheter-related bloodstream infections(P=0.013).Conclusions Conducting critical control point to urological cancer patients carrying PICC catheter for chemotherapy can effectively reduce the incidence of PICC complications.It is suggested that nurses should carry out standardized management for PICC catheter,and pay attention to the complications of patients in chemotherapy.
