超声检查联合γ-干扰素酶联免疫斑点法在附睾结核诊断中的价值

摘　　要：目的分析附睾结核患者超声图像特点,并探究超声检查联合γ-干扰素(IFN-γ)酶联免疫斑点法(ELISPOT)检测在附睾结核诊断中的价值。方法纳入本院2018年6月-2019年2月经手术病理确诊的附睾结核患者46例及同期因附睾隐痛或阴囊坠胀不适来本院就医的非附睾结核患者20例为研究对象。所有患者均行多普勒超声检查及IFN-γELISPOT检测,分析两种检查方法诊断附睾结核的准确度、灵敏度、特异度。结果46例附睾结核病例中,38例经超声诊断为附睾结核。超声检查在附睾结核诊断中准确度为84.85%、灵敏度为82.61%、特异度为90.00%;IFN-γELISPOT在附睾结核诊断中准确度为87.88%,灵敏度为91.30%、特异度为80.00%;超声检查联合IFN-γELISPOT在附睾结核诊断中准确度为90.91%、灵敏度为97.83%,特异度为75.00%。结论超声检查可显示附睾结核的病变部位,附睾呈低回声、非均质肿大或伴睾丸低回声;超声检查联合超声IFN-γELISPOT在附睾结核诊断中具有较高的诊断价值,对疾病早期诊断具有重要意义。Objective To analyze the characteristics of ultrasound images of patients with epididymal tuberculosis,and to explore the diagnostic value of ultrasonography combined with interferonγ(IFN-γ)enzyme-linked immunosorbent spot(ELISPOT).Methods A total of 46 patients with epididymal tuberculosis and 20 patients with non-epididymal tuberculosis underwent Doppler ultrasonography and IFN-γELISPOT detection to analyze the accuracy,sensitivity,and specificity of the two methods.Results The accuracy,sensitivity and specificity of ultrasonography was 84.85%,82.61%and 90.00%,respectively.The accuracy,sensitivity and specificity of IFN-γELISPOT was 87.88%,91.30%,and 80.00%,respectively.The accuracy,sensitivity and specificity of ultrasonography combined with IFN-γELISPOT was 90.91%,97.83%,and 75.00%,respectively.Conclusion Ultrasonography can show the lesion site of epididymal tuberculosis.The epididymis is hypoechoic,heterogeneously enlarged or accompanied by hypoechoic testis.Ultrasonography combined with IFN-γELISPOT has a high value in the diagnosis of epididymal tuberculosis,especially in the early diagnosis of the disease.
