摘  要:目的 通过AB型类孟买血型的血清学检测和基因检测,探讨准确鉴定该血型的方法和输血治疗策略。方法 采用微柱法和试管法进行ABO血型鉴定,使用试管法鉴定H、Rh和Lewis抗原,通过吸收放散实验和唾液血型物质实验检测A/B抗原,结合分子生物学方法进行基因测序,综合判定血型。采用血清学方法进行交叉配血。结果 血清学上证实为ABmh分泌型,ABO基因型为A1/B型,FUT1直接测序发现有以下纯合突变:881-882delTT,确认基因型为h2h2型。FUT2基因测序发现有以下突变:390C>T。390位C>T的突变是同义突变。结论 血清学方法可初步鉴定类孟买血型,结合分子生物学方法可准确鉴定该血型。类孟买血型个体输血时应根据不同患者具体情况,选择最合适的输血方案。Objective Through serological test and gene test of AB Mumbai blood group to explore the accurate identification of this blood type methods and blood transfusion treatment strategies.Methods ABO blood type was identified by microcolumn method and test tube method,H,Rh and Lewis antigen were identified using test tube method,A/B antigen was detected by absorption and dispersion experiments and saliva blood type substance experiments,and gene sequencing was performed in combination with molecular biological methods to determine blood group.Cross blood was performed by serological methods.Results The serological results were confirmed as ABmh secretory type,ABO genotype was A1/B,FUT1 direct sequencing reveal the following homozygous mutations:delTT,881-882 and confirm genotype as h2h2 type. FUT2 gene sequencing reveal the following mutations:390C>T.The mutation is synonymous mutation.Conclusion The serological method can preliminarily identify Mumbai blood type.Combined Molecular biology method can accurately identify the blood type. Bombay blood type individual blood transfusion should choose the most suitable blood transfusion scheme according to different patients.
