摘  要:目的探讨低氧诱导因子1α(HIF-1α)和核因子κB(NF-κB)水平评估急性呼吸窘迫综合征(ARDS)患者发生肺纤维化的价值。方法选取86例ARDS患者作为研究对象,根据是否发生肺纤维化将患者分为纤维化组(35例)和无纤维化组(51例)。比较两组患者外周血转化生长因子β1(TGF-β1)、HIF-1α和NF-κB水平;分析外周血HIF-1α水平与NF-κB水平的相关性,以及ARDS患者发生肺纤维化的影响因素;采用受试者工作特征曲线分析外周血HIF-1α、NF-κB水平单独及联合预测ARDS患者发生肺纤维化的价值。结果纤维化组TGF-β1、HIF-1α、NF-κB水平均高于无纤维化组(均P<0.05)。相关性分析结果显示,外周血HIF-1α水平与NF-κB水平呈正相关(P<0.05)。外周血TGF-β1、HIF-1α、NF-κB水平是ARDS患者发生肺纤维化的影响因素(均P<0.05)。外周血HIF-1α、NF-κB水平单独及联合评估ARDS患者发生肺纤维化的曲线下面积分别为0.874、0.866、0.919。结论ARDS患者外周血TGF-β1、HIF-1α、NF-κB水平升高,三者是ARDS患者发生肺纤维化的影响因素,HIF-1α、NF-κB水平对评估ARDS患者发生肺纤维化均有一定的价值。Objective To explore the value of hypoxia inducible factoR1α(HIF-1α)and nucleaRfactoRkappa-B(NF-κB)levels in the evaluation of patients with acute respiratory distress syndrome(ARDS)suffering from pulmonary fibrosis.Methods A total of 86 ARDS patients were selected as the research subjects,and they were assigned to fibrosis group(35 cases)oRnon-fibrosis group(51 cases)according to whetheRpulmonary fibrosis occurred oRnot.The levels of transforming growth factoRβ1(TGF-β1),HIF-1αand NF-κB in peripheral blood were compared between the two groups,and the correlation of HIF-1αlevel with NF-κB level in peripheral blood,as well as the influencing factors foRARDS patients suffering from pulmonary fibrosis were analyzed.The values of HIF-1αand NF-κB levels in peripheral blood alone and jointly foRpredicting the occurrence of pulmonary fibrosis in ARDS patients were analyzed by the receiveRoperating characteristic curve.Results The fibrosis group exhibited higheRlevels of TGF-β1,HIF-1αand NF-κB as compared with the non-fibrosis group(all P<0.05).The results of correlation analysis revealed that HIF-1αlevel positively correlated with NF-κB level in peripheral blood(P<0.05).The levels of TGF-β1,HIF-1αand NF-κB in peripheral blood were the influencing factors foRthe occurrence of pulmonary fibrosis in ARDS patients(all P<0.05).The areas undeRthe curve of HIF-1αand NF-κB levels in peripheral blood alone and jointly foRevaluating the occurrence of pulmonary fibrosis in ARDS patients were 0.874,0.866,and 0.919,respectively.Conclusion The levels of TGF-β1,HIF-1αand NF-κB in peripheral blood are elevated in ARDS patients,and the three are the influencing factors foRthe occurrence of pulmonary fibrosis in ARDS patients.HIF-1αand NF-κB levels exert value on the evaluation of ARDS patients suffering from pulmonary fibrosis to a certain extent.
