强脉冲光联合氨甲环酸治疗黄褐斑临床疗效的Meta分析

摘  要:目的:系统评价强脉冲光(IPL)联合氨甲环酸治疗黄褐斑患者的临床疗效以及安全性。方法:计算机检索Pubmed、EMBASE、Cochrane、中国知网(CNKI)、万方(Wanfang)和维普(VIP)数据库中关于强脉冲光联合氨甲环酸治疗黄褐斑的随机对照实验(RCT)。检索时间为建库至2021年6月。由2名研究者按照纳入与排除标准独立筛选文献、提取资料并评价资料质量,评价的结局指标包括临床治愈率、MASI、PGA、患者自我评估,复发率和不良反应发生率,采用RevMan5.4软件进行Meta分析。结果:共纳入8篇RCT,包括715例患者,其中强脉冲光联合氨甲环酸患者组359例,氨甲环酸治疗组356例。经Meta分析结果显示,相比单独使用氨甲环酸进行治疗,采用强脉冲光联合氨甲环酸对黄褐斑的总有效性高于对照组[OR=3.67,95%CI=(2.56,5.26),P<0.001]。结论:强脉冲光联合氨甲环酸治疗黄褐斑在临床总有效率上优于单用氨甲环酸进行治疗。Objective Intense pulsed light(IPL)is a commonly used clinical method to treat chloasma.To systematically evaluate the clinical efficacy and safety of intense pulsed light combined with tranexamic acid in patients with chloasma.Methods The randomized controlled trial(RCT)of intense pulsed light combined with tranexamic acid in the treatment of chloasma was searched in Pubmed,EMBASE,Cochrane,CNKI,Wanfang and VIP databases.The retrieval time is from the establishment of the database to June 2021.Two researchers independently screened the literature according to the inclusion and exclusion criteria,extracted data and evaluated the quality.The evaluated outcome indicators included clinical cure rate,MASI,PGA,patient self-assessment,recurrence rate and adverse reaction rate.RevMan5.4 software was used for meta analysis.Results Totally 8 RCT were included,including 715 patients,including 359 patients in the intense pulsed light combined with tranexamic acid group and 356 patients in the tranexamic acid treatment group.Meta analysis results showed that the total efficacy of intense pulsed light combined with tranexamic acid on chloasma was higher than that of the control group compared with the treatment with tranexamic acid alone[OR=3.67,95%CI=(2.56,5.26),P<0.001].Conclusion The total effective rate of intense pulsed light combined with tranexamic acid in the treatment of chloasma is better than that of tranexamic acid alone.
