局部皮瓣在面部创面美容修复中的应用及技巧探讨

摘　　要：目的:探讨局部皮瓣在面部创面修复中的应用及其美容修复技巧。方法:选取2019年9月-2021年9月笔者科室收治的184例面部创面修复患者。遵循面部美学分区修复原则,根据创面大小、部位、形状以及创面周围皮肤情况,选择和设计不同的局部皮瓣修复创面,切口尽量沿着面部轮廓线或皱褶线设计。所有皮瓣缝合均采用精细美容缝合技术,分层减张、精确对位。随访观察修复效果,并记录患者满意度。结果:184例患者术后皮瓣均存活良好,伤口一期愈合。随访6~12个月,皮瓣色泽、厚度、质地与周围组织接近,切口隐蔽,瘢痕不明显,面部无不对称,无眼、口、鼻功能和形态改变。184例患者中,满意156例,基本满意20例,不满意8例,总满意度为95.65%。结论:面部创面美容修复与功能重建中应用局部皮瓣具有创伤小、痛苦少、皮瓣厚度、颜色及组织特性能够较好匹配受区等优势。同时,辅以精细美容缝合技术,精心术后护理,能有效减轻瘢痕形成,患者接受度及满意度较高。Objective To explore the application of local skin flap in the repair of facial wounds and its cosmetic repair techniques.Methods Between September 2019 and September 2021,184 patients with facial wound repair were treated in our department.Follow the facial aesthetic restoration principle,the different local skin flap to repair facial wounds were selected and designed according to the size,position,shape of the wound,and the skin condition around the wound.The incision should be designed along the contour line or wrinkle line of the face as much as possible.All the fl aps were sutured with fi ne cosmetic technique,layered tension reduction and accurate alignment.Followed up the repair ef fect and recorded the patient satisfaction.Results All the fl aps survived well and incisions healed in stage one.Followed up for 6-12 months,the repair fl ap and surrounding skin had good similarity in terms of color,thickness,texture,the incisions were hidden and the scars were not obvious.There was no asymmetry deformity in the face,no functional and morphological changes in the eyes,mouth,nose,etc.Among 184 patients,156 were satisfi ed,20 were basically satisfi ed,8 were dissatisfi ed,and the total satisfaction was 95.65%.Conclusion The local skin fl ap was used in the cosmetic repair and functional reconstruction of the facial wound.It has the advantages of less trauma,less pain,and better matching of the thickness,color and tissue characteristics of the skin fl ap with the recipient area.At the same time,it can ef fectively reduce the scar formation with the help of fi ne cosmetic suture technology and careful postoperative care.This method can achieve higher satisfaction of patients.
