经结膜入路睑袋去除联合眶隔脂肪填充矫正泪沟畸形

摘　　要：目的:探讨经结膜入路去除睑袋联合眶隔脂肪填充矫正泪沟畸形的临床效果。方法:收集73例于诊所就诊的睑袋伴有泪沟畸形的就医者为研究对象,采用经结膜入路的方式适量去除眶隔脂肪,再将取出的眶隔脂肪剪碎填充于泪沟凹陷处矫正泪沟畸形,术后3个月进行疗效评价。结果:73例就医者术后睑袋及泪沟畸形得到改善,其中显效46例(63.0%),良好20例(27.4%),一般7例(9.6%),无效0例(0.0%),总有效率为90.4%。32例(43.8%)就医者出现了下睑皮肤瘀斑,5例(6.8%)出现巩膜瘀血,术后均自行消退;4例(5.5%)下睑泪沟脂肪填充处出现结节硬块,处理后恢复。结论:经结膜入路去除睑袋联合眶隔脂肪填充矫正泪沟效果可靠,安全性好。Objective To discuss the ef fect of transconjunctival lower eyelid blepharoplasty combined with orbital fat injection for the treatment of eyelid bag and tear trough deformity.Methods 73 cases of patients with eyelid bag and tear trough deformity were collected.The orbital fat was removed the through the conjunctival approach,and then the fat was fi lled to correct the tear trough deformity.The postoperative ef fect was evaluated 3 months after operation.Results The eyelid bag and tear trough deformity were improved in 73 patients,of which 46 cases were markedly ef fective(63.0%),20 cases were good ef fective(27.4%),7 cases were improved(9.6%),0 cases were inef fective(0.0%),and the ef fective rate was 90.4%.32 patients(43.8%)had ecchymosis on the lower eyelid,5 patients(6.8%)had scleral congestion,all patients subsided spontaneously after operation.And 4 patients(5.5%)had nodular lumps at the area of fat fi lling,which recovered after treatment.Conclusion The method of removing orbital fat through conjunctival and fi lling tear trough with orbital fat is reliable and safe.
