鼻内镜下同期行鼻整形术和鼻中隔矫正术或鼻中隔成形术疗效比较

摘　　要：目的:比较鼻内镜下同期行鼻整形术和鼻中隔矫正术或鼻中隔成形术后患者的鼻通气功能及美学效果。方法:按照随机数字表法将2018年1月-2020年12月笔者医收治的100例外鼻畸形伴鼻中隔偏曲患者分为矫正组与成形组,每组50例,分别于鼻内镜下同期行鼻整形术和鼻中隔矫正术或鼻中隔成形术,术后随访6个月。比较两组鼻通气功能指标[鼻腔通气评分、鼻腔最小截面积(Minimum cross-sectional area of nasal cavity,NMCA)、鼻腔容积(Nasal cavity volume,NV)、鼻腔呼气吸气阻力];美学指标(鼻尖突出度、鼻额角、鼻唇角)及术后并发症情况。结果:术后6个月,两组鼻通气功能指标及美学指标均得到明显改善。成形组的鼻腔通气评分、NMCA及NV均明显高于矫正组,鼻腔呼气吸气阻力明显低于矫正组,差异有统计学意义(P<0.05)。成形组的鼻尖突出度明显高于矫正组,差异有统计学意义(P<0.05)。两组鼻额角及鼻唇角比较差异无统计学意义(P>0.05)。两组术后均未发生鼻腔粘连、鼻中隔穿孔及塌鼻等并发症。结论:鼻内镜下同期行鼻整形术和鼻中隔成形术对鼻畸形伴鼻中隔偏曲患者鼻通气功能及美学效果改善作用更加显著。Objective To compare the nasal ventilation function and aesthetic eff ect of patients after simultaneous rhinoplasty combined with nasal septum correction or nasal septoplasty under nasal endoscope.Methods From January 2018 to December 2020,a total of 100 patients with nasal deformity with nasal septum deviation who was divided into the correction group and the septoplasty group according to random number table method,with 50 cases in each group,and simultaneous rhinoplasty combined with nasal septum correction or nasal septoplasty under nasal endoscope was performed respectively.The patients were followed up for 6 months after operation.The nasal ventilation function indexes(nasal ventilation score,NMCA,NV,nasal expiratory-inhalatory resistance),aesthetic indexes(nasal tip protrusion,nasofrontal angle,nasolabial angle)and postoperative complications were compared between two groups.Results At 6 months after operation,the nasal ventilation function indexes and aesthetic indexes were improved signifi cantly in two groups.The nasal ventilation score,NMCA and NV in the septoplasty group was signifi cantly higher than that in the correction group,the nasal expiratory-inspiratory resistance was significantly lower than that in the correction group,the differences were statistically significant(P<0.05).The nasal tip protrusion of the septoplasty group was significantly higher than that of the corrective group(P<0.05).There were no significant difference between the two groups in the comparison of nose frontal angle and nose lip angle(P>0.05).No complications such as nasal cavity adhesion,nasal septum perforation and nasal collapse occurred in both groups.Conclusion Simultaneous rhinoplasty combined with nasal septoplasty under nasal endoscope had more signifi cant improvement on nasal ventilation function and aesthetic eff ect of patients with nasal deformity with nasal septum deviation.
