摘 要:目的 探讨改良肺部超声评分(LUS)与呼吸窘迫综合征(RDS)新生儿氧合状态和呼吸支持模式的相关性。方法 选取56例RDS新生儿,于出生24 h内和出生第2、3、5、7、10、14、21、28天分别进行超声检查,分析改良LUS与氧合状态(S/F)、呼吸支持模式的相关性;比较前后肺野改良LUS评分的差异。结果 改良LUS与S/F呈负相关(r=-0.635,P<0.001),组内相关系数0.94(95%可信区间0.93~0.95),Cronbach’s α=0.99,组间一致性良好。不需要补充氧气且可自主呼吸的RDS新生儿改良LUS为2(0,4)分,补充氧气、持续气道正压通气、有创通气(标准间歇正压通气和高频振荡通气)RDS新生儿对应的改良LUS分别为6(4,8)分、10(7,12)分、14(12.5,15.5)分。后肺野和前肺野改良LUS评分分别为4.0(3.8,4.1)分、2.2(2.0,2.4)分,差异有统计学意义(P<0.001)。结论 改良LUS与RDS新生儿氧合状态和呼吸支持模式具有相关性,可为临床评估RDS新生儿情况提供参考。Objective To investigate the correlation between modified lung ultrasound score(LUS)and oxygenation status and respiratory support mode in newborns with respiratory distress syndrome(RDS).Methods Totally 56 neonates with RDS were selected,and underwent ultrasound examination within 24 h after birth and at 2,3,5,7,10,14,21 and 28 d after birth,respectively,the correlation between modified LUS and oxygenation status(S/F) and respiratory support mode was analyzed.The difference of modified LUS score in anterior and posterior lung fields was compared.Results The modified LUS was negatively correlated with S/F(r=-0.635,P<0.001),the intra-group correlation coefficient was 0.94(95% confidence interval 0.93~0.95),Cronbach’s α=0.99,and the inter-group consistency was good.The modified LUS of RDS neonates who did not need supplemental oxygen and could breathe autonomously was 2(0,4). The modified LUS of supplemental oxygen,continuous positive airway pressure and invasive ventilation(invasive positive pressure ventilation and high frequency oscillatory ventilation) were 6(4,8),10(7,12) and 14(12.5,15.5),respectively. The modified LUS of posterior lung field and anterior lung field were 4.0(3.8,4.1)and 2.2(2.0,2.4),respectively,the difference was statistically significant(P<0.001).Conclusion Modified LUS is correlated with oxygenation status and respiratory support mode in newborns with RDS,which can provide reference for clinical evaluation of newborn with RDS.
