摘  要:目的:研究骨髓间充质干细胞(MSC)与分化抗原簇蛋白133(CD133)+肾脏细胞治疗急性肾损伤的机制。方法:选取6~8周龄雄性C57 BL/6的48只小鼠作为实验对象,根据不同的治疗方案将48只小鼠分为正常对照组、缺血再灌注损伤(I/R)组、I/R+MSCs组及I/R+CD133^(+)组4组,每组12只。比较各组小鼠术后不同时间点血清尿素氮(BUN)与肌酐(Cr)水平、ATN评分、肿瘤坏死因子⁃α(TNF⁃α)、白细胞介素⁃10(IL⁃10)、肝细胞生长因子(HGF)及骨形态发生蛋白⁃7(BMP⁃7)水平。结果:I/R+MSCs组、I/R+CD133^(+)组术后BUN、Cr水平、ATN评分、TNF⁃α水平均显著高于正常对照组;术后4、7 d的BUN、Cr水平、ATN评分、TNF⁃α水平均低于I/R组,其中I/R+MSCs组以术后8 d最为显著(P<0.05);I/R+CD133^(+)组以术后4 d最为显著(P<0.05)。I/R+MSCs组、I/R+CD133^(+)组术后IL⁃10、HGF、BMP⁃7水平均低于正常对照组,高于I/R组(P<0.05)。与I/R+MSCs组比较,I/R+CD133^(+)组术后BUN、Cr水平,ATN评分及TNF⁃α水平较低,IL⁃10、HGF、BMP⁃7水平较高。结论:骨髓MSCs、CD133^(+)肾脏细胞均可通过内分泌方式调控细胞因子,促进由I/R诱导的AKI恢复,但CD133^(+)肾脏细胞对AKI的修复作用更为显著。Objective:To determine the mechanism of bone marrow mesenchymal stem cells(MSCs)and cluster of differentiation 133(CD133)+kidney cells in the treatment of acute kidney injury.Methods:Forty⁃eight male C57 BL/6 mice aged 6⁃8 weeks old were included as the subjects in the study.According to different treatment methods,all mice were divided into the normal control group,ischemia⁃reperfusion injury(I/R)group,I/R+MSCs group,and I/R+CD133^(+)group(n=12 each).The serum urea nitrogen(BUN)and creatinine(Cr)levels,ATN score,tumor necrosis factor⁃α(TNF⁃α),interleukin⁃10(IL⁃10),hepatocyte growth factor(HGF)and bone morphogenetic protein⁃7(BMP⁃7)levels of mice in each group were compared at different time points after the operation.Results:The postoperative BUN and Cr levels,ATN score,and TNF⁃αlevel in the I/R+MSCs group and I/R+CD133^(+)group were significantly higher than those in the normal control group.The BUN and Cr levels,ATN score,and TNF⁃αlevel in the I/R+MSCs group and I/R+CD133^(+)group were lower than those in the I/R group at 4d and 7d after operation;these were more significant with the I/R+MSCs group at 8d after operation(all P<0.05),and with the I/R+CD133^(+)group at 4d after operation(all P<0.05).The postoperative IL⁃10,HGF and BMP⁃7 levels in the I/R+MSCs group and the I/R+CD133^(+)group were lower than those in the normal control group and higher than those in the I/R group(all P<0.05).Compared with the I/R+MSCs group,the postoperative BUN and Cr levels,ATN score and TNF⁃αlevel were lower in the I/R+CD133^(+)group,whereas the IL⁃10,HGF,and BMP⁃7 levels were higher.Conclusion:Both bone marrow MSCs and CD133^(+)kidney cells may regulate cytokines in an endocrine manner and promote the recovery of I/R induced AKI.Comparatively,the repair effect of CD133^(+)kidney cells on AKI is more significant.
