超声诊断在子宫腺肌症中的临床应用

摘　　要：目的探究对子宫腺肌症运用超声诊断的临床价值。方法选择2019年8月—2020年8月高邮市人民医院诊治的180例疑似子宫腺肌症患者,均予经腹部超声检查以及经阴道超声检查,以手术病理结果为金标准,分析经腹与经阴道超声诊断对子宫腺肌症的诊断价值。结果手术病理证实180例疑似子宫腺肌症患者中子宫腺肌症153例,阳性率为85.00%。经腹超声诊断阳性133例,阴性47例,与手术病理结果比较,一致性一般(Kappa=0.43)。经阴道超声诊断阳性149例,阴性31例,与手术病理结果比较,一致性较好(Kappa=0.84)。经阴道超声诊断准确率、灵敏度分别为95.56%、96.08%,高于经腹超声诊断的73.89%、82.35%,差异有统计学意义(χ^(2)=35.372、17.640,P<0.05)。经阴道超声与经腹超声特异度比较,差异无统计学意义(P>0.05)。结论在子宫腺肌症患者的诊断中,经阴道超声检查相较于经腹部超声检查受外界因素干扰较少,诊断准确率相对较高,能够为后续治疗提供有利依据。Objective To explore the clinical value of using ultrasound diagnosis for adenomyosis.Methods A total of 180 patients with suspected adenomyosis diagnosed and treated in the Gaoyou People's Hospital from August 2019 to August 2020 were selected,and all of them underwent transabdominal ultrasonography and transvaginal ultrasonography.Used surgical and pathological results as the gold standard,the diagnostic value of transabdominal and transvaginal ultrasonography for adenomyosis was analyzed.Results Among the 180 patients with suspected adenomyosis,153 cases of adenomyosis were confirmed by operation and pathology,and the positive rate was 85.00%.Transabdominal ultrasound diagnose showed positive results in 133 cases and negative results in 47 cases,compared with the surgical and pathological results,the consistency was general(Kappa=0.43).Transvaginal ultrasonography diagnose was positive in 149 cases and negative in 31 cases,compared with the results of surgery and pathology,the consistency was good(Kappa=0.84).The diagnostic accuracy and sensitivity of transvaginal ultrasonography were 95.56% and 96.08%,respectively,which were higher than those of transabdominal ultrasonography(73.89% and 82.35%),the difference was statistically significant(χ^(2)=35.372,17.640,P<0.05).There was no statistically significant difference in specificity between transvaginal ultrasound and transabdominal ultrasound(P>0.05).Conclusion Compared with transabdominal ultrasonography,in the diagnosis of adenomyosis,transvaginal ultrasonography is less affected by external factors and has a relatively high diagnostic accuracy,which can provide a favorable basis for subsequent treatment.
